Deputy Health and Disability Commissioner Rose Wall today released a report finding a registered nurse and a rest home in breach of the Code of Health and Disability Services Consumers’ Rights (the Code) for failures in the care of a rest home resident with cardiac symptoms.
The resident, a woman in her late eighties, had a medical history including coronary heart disease and COPD (a lung disease). She began to experience pain in her shoulder and breast. Early the next morning, the caregiver became concerned and called the on-call registered nurse at her home. The nurse instructed the caregiver to record the woman’s blood pressure every hour and to call back

The nurse and the caregivers discussed the womanâs condition by telephone on two further occasions, but the nurse did not assess her in person.

released a report finding a district health board (DHB) in
breach of the Code of Health and Disability Services
Consumers’ Rights (the Code) for failures in the care of a
three-year-old boy with pneumonia.
The boy became
unwell and was seen twice at a medical centre. On the second
presentation, the GP kept the boy for observation until the
clinic closed, and then transferred him to an urgent care
clinic. The urgent care clinic then referred him to the
public hospital.
The boy was transferred to the
children’s ward when he arrived at hospital. Records of

Deputy Health and Disability Commissioner Rose Wall today released a report finding a podiatrist in breach of the Code of Health and Disability Services Consumers’ Rights (the Code) for failing to comply with professional and ethical standards in the care of a woman requiring podiatric surgery.
The woman consulted the podiatrist for advice and treatment about a bunion deformity on her left foot. At the time of events, the podiatrist had restrictions on his practice that allowed him to perform podiatric surgery only under the supervision of a podiatric or orthopaedic surgeon, and required him to provide the Podiatrists Board of New Zealand with monthly reports of the surgery he performed.

today released a report finding a registered nurse in breach
of the Code of Health and Disability Services Consumers’
Rights (the Code) for failures in the care of a man at a
residential mental health facility.
The nurse was
rostered to work an overnight shift, from 11pm until 7am the
following morning. As part of his overnight duties, he was
to perform three site checks to ensure that residents were
on site during the night.
The nurse performed the
three site checks at 12.00am, 3.00am, and 6.00am. During the
3.00am check, the nurse performed the check by looking
