Deputy Health and Disability Commissioner Kevin Allan
today released a report finding a counsellor in breach of
the Code of Health and Disability Services Consumers’
Rights (the Code) for failing to maintain professional
boundaries with a woman in his care.
The woman
attended a counselling session with the counsellor and at
that appointment they discussed her difficulty with finding
housing.
The counsellor offered the woman a room to
rent at his home on the basis that she could have no further
counselling sessions with him if she accepted. The woman

released a report finding a rest home in breach of the Code
of Health and Disability Services Consumers’ Rights (the
Code) for failures in its care of an elderly man.
The
man had a complex medical history and was a resident at
Armourdene Rest Home (Armourdene), owned by Wilding
International Limited.
Early one morning the man had
an unwitnessed fall in his room. A caregiver called the
on-call registered nurse and monitored the man. Later that
morning another caregiver called the on-call registered
nurse and the man was transferred to hospital. He was

Deputy Health and Disability Commissioner Rose Wall today released a report finding an assisted living facility and nurse in breach of the Code of Health and Disability Services Consumers’ Rights (the Code) for failures in the care of a woman who suffered burns.
The woman lives with spastic quadriplegia and does not communicate verbally. While she was resident of the community home owned by St John of God Hauora Trust, there were several oversights in the management of her continence products, including the delay in monitoring and replacing them for 12 hours.

Health Board (BOPDHB) in breach of the Code of Health and
Disability Services Consumers’ Rights (the Code) for
failings in its care of a young man requiring mental health
services.
The man was admitted to the Mental Health
Ward for two nights, with suicidal ideation. After he was
discharged, he was seen regularly by a psychologist from the
DHB, and his care was discussed at multidisciplinary team
