A woman in her 30s buried two unborn babies after her GP prescribed medication that could cause severe and life-threatening birth defects.
She was also diagnosed with drug-induced Cushing s syndrome after multiple doctors ignored warnings from specialists about overuse of a skin cream and failed to inform the woman of the risks.
Today, her main doctor has been found in breach of the Code of Health and Disability Services Consumers Rights for failures in the woman s care.

released a report finding a rest home provider and two
registered nurses in breach of the Code of Health and
Disability Services Consumers’ Rights (the Code) for
failures in the care of an elderly resident.
The man,
aged in his eighties at the time of events, had multiple
existing health conditions and was prescribed morphine for
worsening pain. One morning, a registered nurse and two
caregivers were on duty. None of these staff members had met
the medication competency requirements set out in the rest
home’s Medication Management policy. Medication competency
Wall said the patient, a man in his 80s, had multiple existing health conditions and had been prescribed morphine for worsening pain. During one shift a nurse drew up 2.5ml of subcutaneous morphine solution without checking the prescribed route of administration or calculating the dose, after being told the man appeared to be distressed. “The subcutaneous morphine solution administered to the man contained 25mg of morphine, which exceeded the maximum quantity prescribed by a factor of five,” the report said. The solution was administered orally in the morning, in the afternoon it was reported the man was unresponsive, and he died that night.
