ACCIDENTS of all sorts frequently occur in the country. Accidents or failures should be a learning opportunity. This is done by investigating or finding the reasons for failure, which is also called root cause failure analysis.
Root cause failure analysis is a part of an investigation of accidents/failures of aircraft, trains, or any engineering system. On the one hand, it helps to improve the working of a system, while, on the other, it promotes a culture of accountability.
Although every accident at the national level needs to be investigated, major air crashes and train accidents need more attention, because the performance of our national organisations, such as the Civil Aviation Authority (CAA) and Pakistan Railways, cannot be compromised.

Pakistan is the only country in the world to have increased the mangrove cover, a recent study has revealed. The study conducted by the Worldwide Fund for Nature-Pakistan in collaboration with Institute of Space Technology and Society for Environment and Mangrove Protection and Welfare Association shows that Pakistan has increased the area under mangrove trees over the past 30 years.
In 1990, mangroves were spread over an area of 477 square kilometres and now they cover an area of 1,463 square kms a significant increase of 300%. This is indeed an achievement considering the fact that mangrove forests across the world are disappearing. Since 95% of the country’s mangrove forests are in the Indus delta in Sindh, the credit for the spectacular achievement goes to the provincial government and non-governmental organisations. PESHAWAR: Putting an end to the growing speculations about the process, the Khyber Pakhtunkhwa government on Friday notified appointment of vice-chancellor of eight public sector universities for a three-year tenure. The notifications were issued Friday evening after formal approval of the summary by Governor Khyber Pakhtunkhwa Shah Farman Khan, who also serves as chancellor of public sector universities.
The summary moved by the Higher Education Department after the list was finalized by the Academic and Search Committee was approved as it is by the provincial cabinet and then by the governor. Thus all the top candidates have been given the task of leading the universities concerned, reliable sources told The News.

The mangrove cover on Sindh s coastal belt, more precisely in the Indus delta, has increased by 986.36 square kilometres (km) during the past 30 years, growing by an average of 3.74 per cent annually, a study has revealed.
Jointly conducted by the Institute of Space Technology, World Wide Fund for Nature-Pakistan (WWF-P) and Society for Environment and Mangrove Protection Welfare Association, the study reviews the growth of mangroves from 1990 to 2020 in five geographical pockets, classified as the Indus delta, Sandspit, Sonmiani Khor, Kalmat Khor and Jiwani.
