CASE A 44-year-old man presented to an outpatient spinal orthopedic clinic for severe lower back pain, possibly related to a scoliotic spinal deformity diagnosed in childhood. History The patient reported severe posterior axial lumbar back pain without radiating leg pain, numbness, or tingling. He denied lower extremity weakness. His past medical history included obesity, autism spectrum disorder, and bilateral sensorineural hearing loss. He denied a family history of scoliosis. He had attempted time, rest, heat, anti-inflammatories, and muscle relaxants without symptomatic improvement. Physical examination A detailed examination revealed an observable scoliotic deformity with left lumbar prominence during forward bend. The entire spine was nontender to palpation in midline. Motor examination was fully intact (5/5) in all muscle groups of the bilateral lower extremities. Sensation was intact to light touch throughout the bilateral lower extremities.
