Gregory Jacobs, 57, was found unresponsive on April 22 in a hotel room in Tampa, Fla., the Hillsborough County Sheriff’s Office reported to the medical examiner.
The Hillsborough County medical examiner’s initial case summary report, released Thursday, lists his cause of death as accidental, the result of “combined toxic effects of fentanyl, ethanol, and methamphetamine.”
