Brief description of the problem
Intrapharm Laboratories Ltd has received a number of complaints that the product consistency is lumpy. The investigation has confirmed through review of the retained samples that the consistency of the complaint samples are similar and the retained samples for this batch show the same homogeneity issue.
Advice for healthcare professionals
Stop supplying the above batch immediately. Quarantine all remaining stock and return it to your supplier using your supplier’s approved process.

Ankors tested the light pink powder substance and found high concentrations of Fentanyl and Benzodiazepines.
“High risk of overdose with prolonged sedation, may not respond to Naloxone, continue to give breaths and seek medical attention,” said Interior Health.
Ankors has offered the following tips and guidelines to protect the public:
Get your drugs checked
Avoid using drugs and alcohol together
Don’t use alone
Start with a small amount
Go slow
Call 911 immediately if something doesn’t feel right
Give rescue breaths and Naloxone if someone has decreased breathing or other signs of an overdose

Impact of Covid crisis in Cornwall: hate activity and drugs

Increased anti-social behaviour, alcohol and drug problems, hate activity and abuse have all been identified as problems in Cornwall as a result of Covid and lockdown.
The Safer Cornwall Partnership – which brings together representatives from Cornwall Council, Devon and Cornwall Police, the NHS and probation service – have provided details of new risks that have been highlighted in Cornwall.
They were included in a report which went before Cornwall Council’s neighbourhoods overview and scrutiny committee on Thursday.

Brief description of the problem
Fairmed Healthcare GmbH has informed us of a discrepancy on the product packaging for the above batches. The discrepancy relates to the incorrect spelling of one of the active pharmaceutical ingredients printed on one side (end flap) of the packs in error. The incorrect end flap states that the active pharmaceutical ingredient is ‘carbiopa’ instead of ‘carbidopa’, (see Annex 1 in the attached document below). The other product details on the pack including the name, strength and pharmaceutical form of the medicine are correct on all other sides of the carton.
Advice for healthcare professionals
Healthcare professionals are advised to exercise caution when dispensing the product. There is no risk to product quality as a result of this issue, therefore the associated batches are not being recalled at this time.
