Which is primarily city employee, pensions and healthcare costs and wages that are rising over time. In order to cover those it robs funds for vulnerable populations and voter priorities. These are priorities that voters have chosen to prioritize in good and bad Economic Times. We should not deprioritize them when the economy goes down and the need is the greatest and it treats all setasides alike, even those that rise and fall with particular Funding Sources and where is the provision that says we do not need to add new Police Officers when the deficit is large. Even that setaside happens regardless of cost. The general fund return is very bad policy and open to a couple of types of abuse, allowing city officials to undermine the priorities to not incumbent funds and allocate funds as the year of the end approaches be ....

I just want to say, i agree that setasides are not a perfect policy mechanism but the reality is we work within the flawed system we have. One of the only tools for poor and marginalized communities right now is setasides to get resources for the services that have been historically underfunded. The dignity fund helps fund among other things for seniors to stay in homes and not be evicted and not displaced and end up far away from San Francisco and the communities we know. Were doing a Needs Assessment for the dignity fund right now. It takes time to figure out how to spend the money in a way thats responsible and best for our communities. Your other point was about really the Community Providers and the type of care they deliver. You might have practiced inside and outside the va. I am note going to paint everyone the same picture. It happens to us a lot. I think there are a ton of Community Providers that are delivering extra care. Hopefully strenten them so you dont need to provide. We can provide them in house, timely and state of the art. Thats what i mean for certain things like spinal cord injury we need to make sure they are there to support our patients. At the same time for things that we think are provided at high quality in the community and maybe dont require that expertise as much like a quick exam, a colonoscopy and how do we make sure they are in our network. Something thats near and dear to me as one of those who represent a Success Story of a great specialized care system, spinal cord injury and disease system of care. Before i get into exactly what the topic is, though, i want to ask a couple questions. What are some of the reasons why one becomes a clinician . Why did you choose to become a healer . Money . Raise your hand if its money. Bunch of millionaires in here, right . Social status. Everybody respects a doctor. Job security. Youre always going to have people focused on getting better, so you need people in those industries. And if youre a healer, youve got some job security if you do it well enough, and its probably a combination of all of those. But again, from the perspective of the patient, im willing to bet that somewhere on that list of what brought you into your respective discipline was a sense of compassion. What is compassion .
