It is unforgivable that there was a lack of planning, a lack of preparation and a lack of education. As the cdc director heads to the hill for a grilling, should the agency have been better prepared . If i were writing the papers, that would be my headline we need help. Good morning. For the first time, one of the nurses who works at the dallas hospital under fire for not having proper ebola protocols in place. She is speaking out publicly. Listen to her say what happened when infected patient Thomas Eric Duncan came in for treatment . It was a little chaotic scene. Our Infectious Disease department was contacted to ask what is the protocol, and their answer was, we dont know, were going to have to call you back. We never talked about ebola and we probably should have. You never talked about it prior to mr. Duncan arrive . No. We never had a discussion. They gave us an optional seminar to go to. So they had a seminar they could go to. The hospital wasnt the only one facing questions, though. Today the cdc is under fire after it gave a nurse who treated duncan the okay to fly, even though she reported a low grade fever. The cdcs response after the fact, she shouldnt have gotten on the plane. Now there are fears more than 130 passengers may have been exposed, they were put at risk. This morning multiple schools in two different states are shut down out of precaution. First lets get to where we are in this ebola story. That infected nurse, amber vinson is being treated at Emory University hospital where they have the special bio containment unit. This is after concerns that there may not be enough medical staff in dallas, especially if nurses walk out. But that same hospital is still treating another infected nurse, nina pham. Both women had contact with the ebola patient who died last week. As we mentioned, that dallas hospital is taking a lot of criticism for not having proper protocols in place. Lets bring in senior medical correspondent Elizabeth Cohen who has been there on the ground from the beginning. Elizabeth, the hospital issued a new statement this morning addressing some of the specific allegations from the nurses union, whats the hospital say. Reporter the hospital is saying these allegations dont match the medical record. They say proper protective equipment was used. They say tape was not wrapped around the nurses necks to protect them. They say the only time it may have been used is if the protective gear was too big and so they used tape to synch it up. Elizabeth, stay with us. I want to bring in cnns chief medical correspondent dr. Sanjay gupta outside emery hospital where vinson is being treated. Sanjay, what do you know about her condition there this morning . Reporter she got here late last night. We expect well get an update sometime soon. We know you probably saw the video of her walking off the plane, getting into the ambulance, obviously a good sign, ana overall. Thats the building where shes going to be treated. We know its the same team that treated dr. Brantly and mrs. Write writebol. As you know, ana, theyve had a pretty good track record. Two patients, dr. Brantly and mrs. Writebol. A third patient is doing well, expected to be discharged sometime soon. Based on what you know, sanjay, what is the difference . We know this is a special bio containment unit. The people have expertise in treating highly Infectious Diseases. What are they doing differently that hasnt happened in dallas . I dont know that theyre doing anything differently as much as theyve just been trained really well. Theyve done the training, the dils over and over again. We know that when it comes to the protective equipment, i dont know if youve seen some of the images there, but the protective equipment theyve used to protect the Health Care Workers goes above and beyond what we saw the protocols were from the cdc, and it sounds like it was used in dallas. Thats part of it in terms of the confidence. It makes a big difference. Keep in mind, ana, theres no magic potion or some cutting edge treatment offered at emery that is not offered in dallas. Experimental medications can be given in dallas and was given to mr. Duncan, a blood transfusion from a survival of exbowl la can be given here as well as dallas and was. Its more the training and confidence. It has been suggested by other doctors on our air that perhaps instead of selfmonitoring to try to stop the spread of this deadly virus, maybe these people who have the close contact, the Health Care Workers, other people who may be at risk should be given blood tests regularly to see if they might have ebola because that would show up there first before you feel physically the symptoms. What are you hearing about that and what can you tell us . Reporter i think there could be value in that. When it comes to that sort of Screening Test and a rapid one done early, you want to be sure it meets a couple of criteria. That is that its sensitive enough to pick the virus up even when the patient is not yet sick. Its got to be able to do that. If it doesnt do that, you might get whats called a false negative, meaning you think the patient is free and clear and, in fact, theyre not. Youve also got to make sure you dont run into the situation where you get false positives, where, in fact, the test for some reason comes back as a positive test and the person doesnt actually have it. Youve got to make sure its a good test and it still needs to be confirmed at a place like the cdc here in atlanta. All right. Dr. Sanjay gupta. Is that elizabeth . Are you trying to chime in . Dr. Sanjay gupta, thank you for joining us. Frontier airlines sent a memo to employees saying its grounding two pilots and four flight attendance for 21 days after amber vinson flew on 1143. Theyre showing the abundance of caution now. We know she has fullblown ebola at this moment. Were told she is in worse condition than her colleague nina pham being treated in dallas. Frontier Airlines Says its keeping the aircraft out of service and being extensively cleaned. In fact, cleaning it four times over. Professor Arthur Kaplan division of medical ethics at New York University and also joining us former ntsb managing director peter goelz. Well start with you, peter and talk about the airline procedures. We hear Frontier Airlines saying its going over and above cdc guidance. Is this to try to rebuild confidence in the airlines . Have they already taken a hit . I think theres no question its designed to rebuild confidence. The airplane cleaners are really the forgotten people in the airline business. Theyre paid minimum wage. They are minimally trained. Theyre under great pressure to turn these planes around. They are underequipped. This is an area that needs to be addressed. Right now its a gray area on supervision. Nobody oversees what they do, no federal agency. So i think frontier is doing the right thing, but there are broader questions here for the airplane cleaners. People are scrambling to be better prepared, arthur. The cdcs chief is going to be in the hot seat today before congress, at least at this congress hearing. What does the cdc need to do to restore confidence that it has the situation under control because it doesnt look like it right now . Well, you get this constant drip, drip, drip, a patient here, exposures there, somebody coming out of quarantine. I say the first thing they need to do is say, you know what . Were going to have five or ten more cases. Be ready for it. Were going to get more exposures. We have troops in africa, 3,000 of them. One is likely to come back somehow or another getting exposed. Lets be prepared not for nothing to happen. Lets be prepared for managing a few more cases. Theyre not going to no one should panic about that. Theyre under control. The other thing i think he needs to say when questioned is, look, its great to try and shut down the borders and worry about all that. If we dont get on top of this epidemic in west africa, if we dont deploy more people to shut that epidemic down, were going to get more leakage. Out cant stop the thing at the border if theres an enormous festering epidemic in africa. Lets send more there. Were hearing the question how could this woman have gotten on the plane, how did she get the okay to fly, especially since she was in a 21day window of exposure and especially when she had a fever. Peter, the cdc is saying she shouldnt have flown. She says they told her it was okay. Do you think at the airports wereality the point where they need to question every passenger, have you been exposed to ebola . I dont think passengers could answer that question. I will tell you flight attendance and flight crews are concerned, but theyve been through this before with sars, mrs, n 1 h 1. Theyre more concerned, watching carefully. Making sure their aircraft are fully equipped with the equipment including gloves and wipes. Sure theres a level of concern. Those comments from a Conference Call held by the National Nurses united after two caregivers who worked with Thomas Eric Duncan contracted the virus. Now one of the colleagues fears for her own health saying she can, quote, no longer defend the texas hospital that employs all of them. Knowing what i know, i would try anything and everything to refuse to go there to be treated. I would feel at risk for going there. If i dont actually have e bowl larks may contract it there. Joining me now, pam cipriani, the parent of the American Nurses association, a group of professional nurses, three million members. Also dr. Celine gounder. I want to talk about comments we just heard. First, do you think the president should enter feen in some way and, two, why arent nurses being provided with the right garments . Is it a money issue . Ana, i think the president has begun to step up his involvement and looks to the cdc to provide leadership. Were also asking the cdc to clarify and upgrade their requirements so all nurses and front line Health Care Workers can be safe. We want nurses to be able to talk about their the protective equipment they need and speak up. I think the nurse that came forward from texas presbyterian is very courageous. I think we learned during the early days of their exposure perhaps not all the right equipment and procedures were available. We know thats being rectified. Dr. Guunder, the hospital has come out and is prepared to go before congress and say we made mistakes and were deeply sorry. Knowing that theyre admitting to making mistakes in how they handled this and seeing the repercussions, should there be any action taken against this hospital or even the cdc for how its handled it . Well, i think there are a couple of different people who might want to take action against the hospital. You have duncans family, and i think the delay in initiating care for ebola, the twoday delay between when he was discharged from the hospital and when he was readmitted was a critical period of time. With treating ebola, timing is everything. In addition, because the hospital didnt appropriately implement infectious control procedures, they have nurses exposed with a deadly virus. God forbid, if they were to die from that, that would be on the hospital. I want to ask you, pam, about the fear that nurses might be walking out of the hospital in dallas because they still dont feel theyre being given enough protection and there still arent proper protocols in place. What are you telling your members across the country . We would really encourage them to talk to their leadership. Its very important that this be an issue of dialogue and calm, but very targeted action. We cant get responses when we have pickett lines. What we really need to do is have the oneonone conversations with our staff and leadership and management to say here is what we need, here is the education, here is the training, here is the equipment, and we want to be assured that we can safely take care of our patients and also keep ourselves safe. Were really encouraging them to contact their state Nurses Association if they need more resources, but really begin that dialogue. We know many, many hospitals, if not all, have really stepped up their efforts, particularly in light of what weve learned in the last couple weeks. Pam, do you have confidence the nations hospitals can protect their workers against ebola . We have to keep this in perspective. Theres only a few patients presented in the united states. We have to focus on screening being very important, nurses are on the front lines as well as in hospitals and clinics to be able to identify a patient who is at risk. If so, there must be swift action to take that patient into an isolated area and making sure were taking care of them appropriately. Again, following all the guidelines as they are perhaps revised and strengthened to make sure were keeping all of our workers safe. Dr. Gounder, how many more ebola patients do you think we might see . There are estimates that came out today looking at the epidemic in west africa, its about the order of a dozen or two dozen more imported cases and the affected Health Care Workers, since those are the people most likely to be infected by somebody coming over. Remember, the epidemic in west africa is spiraling out of control. Right now were dealing with 1,000 new cases per week. In two months, 10,000 cases per week are what the estimates show. If thats the case, the number of importations are going to continue to rise. I know weve talked about this, but i do want you to remind us why it wouldnt make sense, according to some Health Care Professionals to stop flights to and from africa to the u. S. First of all, there are no american carriers that fly direct between west africa and the u. S. People are traveling, as did Thomas Eric Duncan through other cities. He went through brussels. You would have to block not just flights from africa but ports of entry in europe. So that becomes very lo gist tickly problematic and has a huge impact. In addition, when you start to do things like banning travel, people who are desperate will find a way, but theyll travel under the radar, and if they do develop symptoms of ebola, will be less likely to come forward for medical attention because theyll be afraid of deportation, detainment, et cetera. Two Research Facilities are frantically testing vaccines on humans for the first time. We got access to one of them. An urgent need to find a vaccine for ebola. In this u. S. Army laboratory, vials like this contain the ingredients. Its one of two ebola vaccines being tested in the u. S. For the first time on humans. Can the vaccine being tested here stop this outbreak . It depends on how fast we can get this particular product through the regulatory pathway so it can be used in efficacytype trials. Right now we have to establish that its safe. If its used, can it be used to prevent people from getting the ebola virus and treat people who already have it . The majority of studies post exposure, animals that have been exposed and treated, we looked at studies that looked at pre exposure. They were given the vaccine and then exposed. Both of those were good results. So we are cautiously opted mystic. Will it work in humans . To be very realistic. Most medicines and vaccines even that look great in animals dont pan out in the long run. Even as they rush these vaccines through trials, there are serious questions over why its taken this long, even though weve known about the ebola virus since 1976 there are no approved ebola Vaccines Available to the public. Why . Experts say ebola outbreaks until now havent been widespread enough. Its not on the order or it hasnt been until recently of a problem like malaria, hiv or tb in terms of how people have prioritized investment in vaccines. Reporter how soon could these vaccines be ready . The doctor says theyre moving the vaccine through testing paces as quickly as possible. He says even under the best of circumstances t vaccines may not be ready for public use for several months. They have to make sure those vaccines are safe. Ana . Brian, could they test these vaccines in country where its at its worse snt. Probably not. Officials continue to say there is a very low risk anyone was exposed to the virus on those flights, but theyre still taking those precautions. He is the american doctor who contracted ebola while he was caring for patients stricken by this virus in liberia. He was treated in emery hospital in atlanta where sanjay is reporting from. He is virusfree. He has donated his own blood several times now to help treat other american ebola patients. He spoke to Anderson Cooper about his recovery as well as his thoughts on whats happening in dallas with the Health Care Workers contracting the virus. I dont know when ill say i feel back to normal, but my strength, my stamina, my energy are improving a lot. I feel a lot better than i did even two weeks ago. Health care workers go into this profession to serve people, to relieve suffering, to cure disease, to come alongside people in the worst times of their lives, and now here we have a Second Health care worker in dallas who was doing just that for a patient who was suffering greatly. Until this epidemic is stopped in west africa, it will continue to be a global problem. There have been suspected or confirmed cases in united states, spain, brazil had a suspected case. Were talking three continents outside africa that have already been affected by this epidemic, this outbreak. The answer is not simply close the borders and let them deal with it themselves. I think theres a lot of irrational fear about ebola spreading to the united states. So good to see him looking so healthy and able to articulate like that. Theyre visually looking at these people, they are the ones doing the temperature checks. If they see something that doesnt look quite right, they flag a cdc representative who will be present, many of them present at all these airports. The cdc representative takes over, whisks that person off to a quarantined center which is right here on airport grounds, gives them a closer look, a more detailed examination, and if they determine that person needs to go to the hospital, fire rescue emergency teams are here at the airport. They take over and they transport that individual to the hospital. That is a process, thats how its going to work. Spoik with the people here from dulles today, so far they have not received any people from that region so they havent had to go through that process just as yet, but they say theyre trained and ready to go when those individuals arrive.
