Staff members found 232 refrigerated vials of the Pfizer vaccine to be outside of the required temperature control range. After an investigation, it was determined that the 1,160 doses were spoiled because the power supply for a refrigerator they were in was accidentally turned off. The agency calls it an isolated incident caused by human error with no impact on patient safety. The vials were destroyed. The Health Care District says it immediately implemented additional layers of quality control measures in its vaccine cold chain management to maintain the integrity of all vaccine supply moving forward.
