Daisy Wood died of pneumonia aged 84 on February 21, 2019 after being rushed to hospital from St Catherine Care Home in Leytonstone. East London coroner Nadia Persaud found that staff’s failure to “adequately observe” Ms Wood meant she was unresponsive by the time a doctor was called to see her. Ms Wood was prescribed the anti-anxiety medication Lorazepam earlier that month, after being physically assaulted by a member of staff at the care home caused her psychological distress.

The parents of a newborn baby who died aged just three days old called 111 three times before an ambulance was sent – and were given incorrect resuscitation advice, an inquest heard. Jordynn Edwards-Jeffers died at Whipps Cross Hospital on February 10 2019 after she stopped breathing at her home. A post-mortem was unable to determine the exact cause of her death. Jordynn’s parents called 111 at 9.48am that morning because she was not feeding but, after a series of errors, an ambulance was not dispatched until 11.23am, during their third call. An inquest held on February 15 at Walthamstow Coroner’s Court heard that, since her death, the London Ambulance Service has completely overhauled how 111 deals with newborns.

And the son of Renee Vincent, 83, spoke of his disquiet at how long it took staff to tell him she was in hospital. Mrs Vincent was admitted to Whipps Cross Hospital on August 31 last year after carers at Forest View Care Home in Walthamstow found her in her room bleeding from a large cut on her head. She died at hospital on September 6, having suffered a brain hemorrhage, but doctors were unsure if her injury caused the bleed or if a spontaneous bleed caused her to injure herself.
