so-called murky chain of command. mark mizetti is with us. i want to start with what happened. how did this jordanian double agent manage to get close enough to kill seven employees, especially in afghanistan where most of us assume that security would be extra tight? right. the cia internal investigation into the incident documents a litany of breakdowns. there were lax security procedures at the base in afghanistan. there were also some early warnings that were missed. a few weeks before the actual attack, the cia station in jordan did get some concerns from jordanian intelligence that the man who ultimately carried out the attack might have been a double agent. these were presented by the cia, not as hard evidence but more of concerns. these concerns were never passed to higher to superior
