Trillium Community Health Plan has partnered with Unite Us, a leading technology company connecting health and social care services, to further enhance the health plan's ability to collaborate with providers and local community-based organizations to provide integrated health care services for Oregon Health Plan (OHP) members in Lane, western Linn, western Douglas, Clackamas, Multnomah, and Washington counties.
The Unite Ohio platform, managed by New York-based technology company Unite Us, was launched in 2020 in partnership with MetroHealth. Cleveland Clinic joined last year, and with UH's partnership, all three systems now are partnering with the network that connects health and social organizations and facilitates referrals among them.

University Hospitals says this new partnership will connect patients across Ohio with various resources for help with things like food, housing and transportation.
