Calhoun County logo BATTLE CREEK, MI (WNWN/WTVB) – The Calhoun County Public Health Department says a vaccination request form is now available on the Calhoun County website for essential front-line organizations that meet Phase 1B criteria. The forms are available at calhouncountymi.gov/covidvaccine under Local Information. The department has released an online form strictly for agencies or organizations that meet the qualifications of Phase 1B as is determined by the Michigan Department of Health and Human Services. The form is not intended for individual submissions by front-line workers. Once the Public Health Department receives the request and agrees that your organization meets the criteria, they will contact you to schedule a clinic with your agency.

That includes seniors over 65, teachers, corrections officers, homeless shelter staff, critical infrastructure staff, food manufacturing, veterinarians, court staff, public transit workers, grocery store workers and more. The Calhoun County Public Health Department is asking for a point of contact of the organization to fill out the form on behalf of the organization they represent.
