Ligature risks and blind spots were identified in areas used by patients who had been secluded, risking the safety of people prone to self-harm.
And on at least five occasions, CCTV captured staff sleeping while they were supposed to be monitoring vulnerable adults.
Inspectors said the hospital was short-staffed, heavily dependent on agency workers, and lacked essential equipment, including for resuscitation.

A mental health patient was pushed to the floor as hospital staff used disproportionate and unauthorised techniques , health inspectors said.
The Care Quality Commission (CQC) inspection took place after concerns were raised about patient safety at St John s House in Palgrave, Suffolk.
It was rated inadequate and banned from admitting new patients.
Priory Healthcare said it had taken immediate action to deliver improvements .

SMITHFIELD – Town Engineer Kevin Cleary anticipates increased development in Smithfield’s Economic Growth Overlay District after local landowner Ronald Caniglia agreed to an easement allowing the extension of sewer and water lines on his property at 167 George Washington Highway.
The Town Council last week approved an easement agreement where Caniglia will pay for the extension of sewer and water lines on his property to Route 7.
Cleary said the town will see savings of more than $250,000, paying only administrative fees and sharing impact fees from future developments until the project cost is reimbursed.
Cleary said the “planets aligned” for the deal, which will cost the town “very little” in staff time at Town Hall and the Smithfield Water Supply Board.
