Questions are being asked over serious problems which have emerged since Aintree Hospital merged with the Royal Liverpool Hospital. In the past month it was reported how thousands of patients were left without follow up appointments in Aintree s gastroenterology department. The ECHO revealed last month how several patients suffered serious deterioration in their vision after appointments failed to be booked or were missed in the ophthalmology department. Now the ECHO can also reveal how NHS Improvement (NHSI), the national arm of the NHS dealing with rectifying failure and improving quality, reviewed a number of so called never events at Liverpool University Hospitals NHS Foundation Trust.
