Who can report fraud?
Anyone can report fraud, waste or abuse. Texas Health and Human Services is required by law to refer suspected provider fraud to the Office of Inspector General.

Call 800-581-1790 to report healthcare fraud. You also can report it online through the Medicare and Medicaid Fraud Reporting Center.
Nicole Guerrero oversees the Texas Health and Human Services system’s Office of Audit and Compliance.
Before taking on the role, Guerrero served as the division’s chief deputy director. She was also the Texas Health and Human Services Commission’s Internal Audit director before transformation merged five agencies into two.
Before joining HHS in 2014, Guerrero worked as an audit manager for the Texas State Auditor’s Office. She has 26 years of audit experience, including 23 years in audit management.
Guerrero has a Bachelor of Science in Accounting from Xavier University in New Orleans and a Master of Business Administration from Texas State University in San Marcos. She is also a Certified Internal Auditor.
