The project is called the National Implementation and Dissemination for Chronic Disease Prevention.
The CDC awarded the National WIC Association (NWA) funding to incorporate this project on the local level.
On the local level, the project is called the Community Partnership for Healthy Mothers and Children..
The purpose of the coalition is to implement population based strategies in the community to strengthen and improve the communities' health..
The focus is to reduce preventable chronic disease by improving access to healthy foods..
In addition, the coalition will work to reduce and prevent chronic disease by effecting access to disease management services.
