Detailed description is Our history dates back to 1988 when the HIV/AIDS Support Group of Sudbury was formed to provide bilingual services, offer assistance and facilitate access to health care to Persons Living with HIV/AIDS and to their families.
In 1996, the Support Group opened Maison La Paix, the only supportive housing program (4 beds) for Persons Living with HIV/AIDS in Northern Ontario.
In March 2005, Maison La Paix received approval from the Ministry of Health and Long-Term Care (MOHLTC) to serve generic palliative residents.
Following a successful capital campaign that raised $4.1 million, the Sudbury community residential hospice opened its doors in September 2008, at the same time that Maison La Paix closed its operations.

Situated in a serene and peaceful environment on Bethel Lake, our unique architecture provides a residence which looks and feels like home.
It provides individuals with privacy yet open access to common spaces to interact with other residents and families..

Between September 2008 and December 2015, over 1,100 residents have lived their last journey at the Hospice, surrounded by family and friends, as well as caring staff and volunteers.
Of that total, 64% of residents came directly from home, 29% from hospital, 5% from the Emergency Department and 2% from long-term care facilities; 89% had a cancer diagnosis and 69% were over the age of 65.
The average length of stay is currently 21 days, with an occupancy rate of 94%..

In April 2013, the Hospice received funding from the North East Local Health Integration Network (NELHIN) for a Shared Care Team (SCT).
While collaborating with clients’ current health service providers, the SCT provides integrated, multi-disciplinary HPC consultation and education.

In April 2014, Warmhearts Palliative Caregivers integrated with MMH, and the program is now called Visiting Hospice Service (VHS).
Warmhearts began in 1986 as the Sudbury Regional Palliative Care Association.
It received its charitable status in 1987, and the first Volunteer Visiting Hospice Service was developed in 1989.
The VHS matches trained volunteers with clients, and their families, who are living with a life-threatening or terminal illness.
MMH offers mandatory training to visiting and residential hospice volunteers in both official languages..

In September 2014, MMH launched a Pediatric HPC Pilot Project to admit and care for a child between the ages of 12 to 18 years with a cancer diagnosis.
The first youth was admitted in January 2016..

In June 2015, the NELHIN Board of Directors supported MMH’s request for the designation of its services under the French Language Services Act.
At the time of publishing this booklet, the designation proposal was in the hands of the Ontario Office of Francophone Affairs..

MMH also offers education and training placements to over 40 students each year from post-secondary institutions (medicine, nursing, personal support, social work, etc.)..

On December 22, 2016, the Hospice received a MILESTONE donation of $1 million from local family and business owners Bruce and Sandra McCulloch.
The Sudbury Hospice has now been renamed Maison McCulloch Hospice..

Notre histoire débute en 1988 quand le Groupe d’appui VIH/SIDA de Sudbury est mis sur pied dans le but d’offrir des services en français aux personnes et aux familles affectées par le VIH/sida (interprétation, accompagnement et hébergement).
En 1996, le Groupe d’appui ouvre Maison La Paix, le seul programme avec services de soutien (4 lits) dans le Nord de l’Ontario pour les personnes vivant avec le VIH/sida.
En mars 2005, le ministère de la Santé et des Soins de longue durée (MSSLD) permet à Maison La Paix d’accueillir tous les résidents en besoin de soins palliatifs.
Suite à une campagne de capitalisation couronnée de succès qui a pu prélever la somme de 4,1 millions de dollars, Maison La Paix a fermé ses portes en août 2008 avec l’ouverture de la Maison de soins palliatifs de Sudbury.

Localisée dans un milieu à la fois serein et paisible sur les rives du lac Bethel, la MMH jouit d’une architecture unique qui crée une ambiance de foyer familial.
Le milieu offre à la fois un endroit intime et des aires ouvertes permettant une interaction avec d’autres résidents et leur famille.

Durant la période de septembre 2008 à décembre 2015, plus de 1 100 résidents ont vécu leur dernier séjour à la MMH, entourés de leurs famille et amis, ainsi que d’un personnel et de bénévoles attentionnés.

Depuis, la MMH a développé des programmes et services dépassant la capacité de 10 lits à sa maison de soins palliatifs.

En avril 2013, la MMH recevait du financement du Réseau local d’intégration des services de santé du Nord-est (RLISSNE) pour la mise sur pied d’une Équipe de soins partagés (ÉSP).
Tout en collaborant avec les pourvoyeurs existants des services de santé, l’ÉSP fournit de la consultation et une éducation intégrée multidisciplinaire au niveau des soins palliatifs..

En avril 2014, l’organisme De tout cœur, offrant des services de soins palliatifs à domicile, s’est intégré à la MMH.
Le programme se nomme maintenant le Service de bénévoles visiteurs (SBV).
De tout cœur avait connu ses débuts en 1986 à titre d’Association régionale de soins palliatifs de Sudbury.
Il obtient son statut d’organisme de charité en 1989 et met sur pied le premier programme de bénévoles visiteurs en 1989.
Aujourd’hui, le SBV jumelle des bénévoles formés avec des clients (et leur famille) qui sont atteints d’une maladie incurable ou en phase terminale.
La MMH offre une formation obligatoire en soins palliatifs aux bénévoles résidentiels et visiteurs dans les deux langues officielles..

En septembre 2014, la MMH a lancé un projet pilote en soins pédiatriques dans le but d’admettre un enfant de 12 à 18 ans.
