If you would like to also receive dental services, please fill out the dental consent form.

Once forms are received, services may begin at SBHC..
Oregon State Law requires a parent or guardian signature for mental health services for students under 14 years of age and medical treatment under 15 years of age, with exception of family planning information and sexually transmitted disease service.

SBHC SATISFACTION SURVEY.
Students between the ages of 12 and 19 who visit the SBHC may be asked to complete an anonymous survey after their visit.
The survey asks about the student’s satisfaction and experience at the SBHC, as well as some general questions about their physical and mental health status.
Students can refuse to take the survey and this will not affect their ability to get care at the SBHC.

The Wellness Center is located at the Pendleton Early Learning Center (PELC) and serves all students and families of Pendleton School District and PELC.
