Some hopeful; many disheartening. And then there is the toll on the economy. More than 6. 5 Million People added to the nations jobless rolls last week. Dr. Jon lapook and money correspond Jill Schlesinger are tracking the numbers. We begin with dr. Lapook. There are many pharmaceutical Companies Working around the clock to find a therapy. But as we do that, it is important we embrace science. What do i mean by that . Take hydroxychloroquine. Hydroxychloroquine has shown promise with the coronavirus. It is in the news a lot lately. It is used to treat lupus or rheumatoid arthritis. A few small studies have looked if it is also affective in treating covid19, and the results have been mixed. So with no proven therapies available, where does that leave Health Professionals like me . Thats when science comes in. You need to do wellcontrolled rigorous studies to see if a drug works and whether the benefits outweigh the risks. But the clock is ticking and patients are dying, and doctors like me want to do something before the Clinical Trials are completed. Thats the friction between wellcontrolled studies and what is called empiric therapy, basically trying something to see if it works. Clinicians are closing the kitchen sink at covid19 because patients are so sick. So patients will also receive a cocktail of medications, including hydroxychloroquine, but also perhaps other experimental drugs, to see does it work . If nothing else is available, it is a reasonable thing to do. But as we do this, we must also do the best possible Clinical Trials, so were relying on data, not beliefs. Whats wrong when empiric therapy, especially if the patient is Getting Better . The problem is you cant know whether a drug works unless you compare it to something else. That means seeing if patients who get the drug do better than those not getting it. And at what stage in the infection does it work the best . Mild illness, severe illness . Can it perhaps prevent infection in people who have been exposed to the virus. All of those studies are going on right now. As always, we have to be on the lookout for side effects. Hydroxychloroquine can rarely cause a serious irregular heartbeat in patients with autoimmune disease. It is rare, but perhaps it will go up in patients who are affected with coronavirus. But you can carefully monitor those patients in a controlled study by picking up the side effect early. There are many potential therapies under development, and were rooting for them all. We clinicians want to try something, even if were not sure it works. But this is no time to abandon the Scientific Method of doing careful research. If we continue to embrace science, our best guesses just may morph into affective therapies. This is Jill Schlesinger, with the growing financial disaster for Small Business owners. The government is offering 10,000 disaster loans, which are made directly by the Small Business administration. And there is the Paycheck Protection Program to help companies retain workers and pay for some additional expenses, like rent and utilities, which can be forgiven. To access this money, owners have to go through banks. It has been a frustrating process so far for many employers. And we have not heard back from that application process. And then the second option is the payroll protection loan. So that, we actually havent been able to apply for yet. So i reached out to my bank, and they basically sent out an email saying they dont really have anything in place yet. We reached out to the f. D. A. About the log jam. Great leader, possibly. First of all, dont be in denial of what the true situation was, and get the information from every Vantage Point you possibly can. The second would be learn as much as you can from the peope who are the closest to the work. They will help guide your choices and decisions, and there is genius that exists out there that you need to listen to. But the third is, boy, collaborate as much as you can with others. Communicate. Communicate, communicate, communicate. I think first you have to care. You can pretend you care about the organization, mission, and the people, but if it is not true, over time it will come out, and there will be a difference between what you say and what you do. Now, fear shouldnt put us into inaction. What fear should do is get you to be a motivator, it should be something that causes us to be more cautious but not slower in the way we operate.
