Photo Credit: Northumberland Hills Hospital Several community organizations are helping to provide support to COVID-19 patients in the community (helping to share information, if additional treatment or hospitalization are needed.) The program includes support from nurses at Home/Community Care Support Services as well as NHH Infection Prevention and Control and the NHH COVID-19 Assessment Centre and more. Patients that are diagnosed in the NHH COVID19 Assessment Centre or emergency department (that are able to return home,) can choose to participate in the remote monitoring program. (Primary care doctors can also provide direct referrals, for patients who may have contracted COVID-19, outside of the community.
