Local agencies unite on grant-funded project to reduce health disparities
Finger Lakes Performing Provider System Inc. has received a $250,000 grant from the Mother Cabrini Health Foundation for a program aimed at reducing health disparities. The program, entitled “Reducing Health Disparities through Community Navigation Support,” includes a collaboration with St. Joseph’s Neighborhood Center and Human Services Development to address health-related needs of low-income and vulnerable individuals.
