Faulty signalling due to botched repair works caused the deadly Balasore train accident in India, according to the Commissioner of Railway Safety. The accident could have been avoided if previous glitches in the signalling system had been addressed. The findings will be part of the Central Bureau of Investigation's criminal investigation into the incident, which killed 288 people. The report recommends a review of the disaster-response system and a drive to review signal wiring diagrams.
