Call for pharmacy/public health teams - APhA/CDC CVD Community of Practice Health Equity Project – Due Feb. 15

APhA and the CDC’s Division for Heart Disease and Stroke Prevention are convening a Community of Practice for Advancing Health Equity with Public Health and Pharmacy Partnerships The project will accelerate the implementation of pharmacy-based strategies to advance health equity through initiatives that address racial and ethnic disparities in cardiovascular disease risk factors, prevalence, or outcomes. We are seeking nominations of teams of representatives from public health departments and the pharmacy community.
