Decision to secure supply of clomipramine hydrochloride and mirtazapine tablets

What we’re doing
mirtazapine and
clomipramine hydrochloride tablets following the announcement that Apotex New Zealand Limited (Apotex), the current supplier, is leaving the New Zealand market.
This decision means that:
Mirtazapine 30 mg and 45 mg tablets
A new brand of 30 mg and 45 mg mirtazapine tablets will be funded from 1 August 2021.  This will be the Noumed brand, supplied by Noumed Pharmaceuticals Limited (Noumed).
The Apo-Mirtazapine brand of mirtazapine (supplied by Apotex) will continue to be funded until 1 January 2022. 
From 1 January 2022, the Noumed brand of 30 mg and 45 mg mirtazapine tablets will be the only funded brand of 30 mg and 45 mg mirtazapine tablets until 30 June 2024. ....

GPS data helped a community health service to untangle an employee s web of lies that covered up her failure to check on an elderly client just days before he died.
The support worker lied about attending her scheduled Saturday 90-minute session with the elderly man at his home in 2018, a Health and Disability Commissioner report has found.
The woman was in fact at her own home - around 11km away - at the time of the session, according to GPS data abstracted from an app used by support workers to log their visits with clients. ....

A rest home has been found to have failed an elderly man in its care

The man was transferred to hospital from Armourdene Rest Home in Hamilton, with a fractured hip after a fall.
He died a few days later.
The man s sister-in-law was concerned about the care he received, including the management of his fall, and complained to Health and Disability Commissioner.
Deputy Commissioner Rose Wall says Armourdene refused to provide the relevant information including the man s clinical records.
She says as a result, it breached the Code of Health and Disability Services Consumers Rights. ....

Woman's uterus removed after botched 'routine' procedure, investigation finds


But the pain got worse and the woman eventually called an ambulance that rushed her to hospital.

The doctor provided the woman with a written apology following the procedure.
Emergency department staff found she had suffered a thermal injury to her uterus during the earlier thermal ablation procedure and sent her to the operating theatre.
Surgeons cut out her uterus, fallopian tubes, and part of her digestive tract. She was also given a stoma – a small hole in the abdominal wall to which the bowel is diverted.
Wall found aspects of the services provided to the woman by the doctor were inadequate, and breached the Code of Health and Disability Services Consumers’ Rights. ....
