There have been five breaches of standards after surgical instruments were not properly sterilised in five operations at Dunedin Hospital over a period of almost a year, it s been revealed.

The five standards breaches, which took place between March 31, 2020 and January 31, 2021, include three involving surgical cement; one involving hair; and one involving water in a connector found on or within the instruments or instrument tray once a procedure had started in theatre, Southern DHB chief executive Chris Fleming said today in a statement.
He said in the first four cases the material and/or instruments were removed and replaced before being used on the patient. ....

The four victims of last week’s attack in Dunedin have given their thanks and gratitude for the support they have been shown by Kiwis over the last week.

Police say the victims have received messages of support from both the wider Dunedin community and around New Zealand .
As they come to terms with what happened and begin to heal both physically and mentally, this support and aroha has been greatly appreciated, police said in a statement today.

National leader Judith Collins says several South Island mayors are concerned by the Government’s advanced plans to transfer 50 per cent of the island’s publicly-owned water assets to iwi ownership.

The Department of Internal Affairs presented the Government’s preferred option for water reform to 23 mayors and Ngāi Tahu, Collins said in a statement.
This new mainland water agency would assume ownership of all water assets and some council debt.
Collins said National was of the understanding that four mega-agencies will be set up across Aotearoa as part of the Three Waters proposal to manage water assets. The agencies would be similar to Auckland’s Wa ....

Officers investigating the incident were still calling for witnesses to come forward as they tried to piece together what happened before and during the attack.
