His report, completed Tuesday, said firefighters were called at 5:46 a.m.
The first fire engine to arrive at 5:59 a.m. was unable to drive all the way up the ice-covered, 1,500-foot driveway to the house. It stopped 400 to 500 feet away from the structure.
Vielhauer and Jerry Lewis told firefighters that Mabel Lewis was trapped inside the house, about 10 feet inside the front door. Two firefighters tried to enter through the front door.
"After about 10 feet they found the fire had burnt through the basement ceiling into the first floor of the living room area," Lynch wrote. The firefighters left the house.
