I know you didnt sleep much because of the exciting developments happening here in San Francisco which we want to get to. I also need haircut. I dont think you need to worry about that. Its okay. Its expendable thing at this moment. Lets get to the new symptoms because the cdc just in the past couple of days listed an additional six symptoms for covid19 and thats causing a lot of confusion. People want to know what they are and how relevant they are in terms of all the symptoms that are out there. If you can run through these for me. Totally. Chills, unlike severe chicagos which we call rigors. Its like when you shake with a chill, not regular chills. Muscle aches, headaches, sore throat. Well talk about how to interpret these symptoms is you cant smell or taste properly. Those are the new ones that the cdc has listed. I want to ask you, granted thats been observed in some patient who is have covid19. Arent those more commonly attributed to the flu or the cold. How worried should be be if they spot that in themselves . I think i was worried i looked at the symptoms because it may lead to people being unnecessarily scared or concerned. What we could talk about or the way i look at this is whats the most important thing people are listing. Its still the original symptoms that the cdc reported which is fever. Thats like super common in covid and then you know you have a cough and you cant breathe properly. Those are the three. Then of the ones that they listed, the sixth ones, probably the most important ones is you cant smell and you cant taste. Okay. One of my colleagues actually put all these together really well in super easy to use guide. Walk up wake up in the morning, check your fever and smell your coffee. If you dont have a fever and you can smell your coffee, then youre okay. How common is toe rashes . Its not something we normally see in medicine. People have been reporting it more. An immine coune complex reactio. When they tested the young people with redness of toes, they didnt find any active infection. We see a reaction on the body after the virus is already come and gone. The cdc doesnt list that because its probably not an active symptom. Could be you had it and got over it. We probably need more cases and data. Again, a bit more in kids rather than in adults. Now i want to switch to the topic of the shelter in place, which as you know, the six main counties along with the city of berkeley have gotten together and the Public Health officers decided to extend the shelter in place through may, all of may. This comes when a lot of people were saying we cant take it anymore. People going to the beach, packing the parks. Can you talk about whether the numbers support the need to continue these types of actions because a lot of people think hospitalizations steady, icu steady, rate of climb steady. Whats going on . I know its really, really tough. Its tough for me too. I kind of joked about not getting haircut early on. I think the shelter in place is affecting all of us. My general comment is i trusted our leaders because its really well informed by Public Health experts as to what to do. They really made a really wise decision early on. I think its up to all of us, myself included, to believe them when they dont think were ready and proceed on easing restrictions. We talked about how were doing in the community. Thats one strategy. The other strategy we mentioned is the whole idea of contact tracing. Even if you doinnt have enough tests, you need a work force thats ready to go out there when theres a case and interview all the people around that case and contain the infection. Once you have the work force to do that, a little bit more testing, you can like, really ease restrictions because then you can say, well, now cases are going up because we have a more spens i sense tiitive barometer. Its about dimming the switch. Its not like turning the light on. In the coming weeks well see some easing up. The lower risk activities may be able to resume. Thats what youre talk about. I want to get to some facebook questions from our viewers. Lisa wants to no can someone get reinfected with covid19 . Thats been a topic of interest heating up in the last week or so because the w. H. O. Tweeted this story and they kind of like moderated it a little bit. My thought about reinfection is it depends on how if you get these good antibodies or not. Not everybody will get the good antibodies. We know when we check their antibody response. You dont usually get them. You dont get super sick. You might feel crappy does its not basaffecting your lungs. If you had mild symptoms you may be able to get it again in next season. Thats why we cant feel home free because until we have a vaccine, not everybody will get that antibody response even if you had it. That makes a lot of sense. The harder it hits you, the harder your body fights back and those antibodies, thats what will protect you. Thats what the vaccine is. Vaccine is like getting around that by tricking the body into thinking its getting super invasive infection. Oscar is asking im planning to drive and move in with my girlfriend and her family in texas. What do i need to do in that process to be as safe as possible . I think one of the interesting thing s driving. I think driving is okay if youre not driving with a lot of of strangers. Drooi with Family Member who is you have been living with for a while. You can sit fix feet apart. If youre driving by yoirs, i think thats one of the factors is most people in california have a car and kind of like you take public transportation. Its okay. When you get to texas is a different story. The epidemic in texas hasnt matured yet. With them decreasing restrictions, you may have a bit more disease in that community. As a californian, you would probably be as the best in the country as what to do to keep yoursef safe. Okay. Good advice. Thank you so much. Lets take a short break on air but continue the conversation how about poor fred wilson . What a shame. So soon after retiring. I hear his wife needed help with the funeral expenses. Thats ridiculous he had social security. When my brother died, his wife received a check from social security, all right for 255 the funeral costs were well over 8,000. All right. Were back on air with ucsf Infectious Disease doctor. Youve been busy with on friday ucsf did a transfusion of convalescent plasma of someone who recovered from covid into a really sick patient. Can you talk to us about that process and if its showing signs of being successful . That was really remarkable thing that happened on friday night. It took us all by surprise. Mainly because it happened out of the blue. Many of these things that seem almost miraculous. We had been preparing the system for three weeks or more to accept these donors who were recovered from covid19. They have the antibodies or the good army against covid19 and the idea is you want to put that army of fighting into people who are super sick and havent developed those antibodies with the army yet. Polio, sars, mers, you might remember people got ivig as a shot. It gave you antibodies so when you went to that Street Vendor you were protected from getting hepatitis a. We havent done it too much in the united states. The fda was so encouraging that it gave blan keket license for people to do it. We talked about Actor Tom Hanks and his wife donating plasma. Are they saying come on in and well send it off and collect it for this purpose in. Yes. Blood banks are ready. I think the easiest is if you have evidence that you had covid19 from a test result. You can ask your doctor for that. You have 28 days of no symptoms. You go to San Francisco or any American Red Cross and they will take you. What if youre like my friend . She thinks she had covid but she never got tested . Theres now way you can do that which is to ask your primary care physician or provider to order you a good Antibody Test so the blood banks will accept an Antibody Test from a good company. You talk to your doctor about how confident they feel about their particular Antibody Test theyre offering. If you have a positive Antibody Test and 28 days of no symptoms, the blood banks will accept you. I will also say if they dont, because theyll do a lot of back testing, it doesnt have the antibodies, theyll probably use it for somebody else who is not covid19 because these are y universal products. Have you sign an improvement in the patient or is it too soon to tell . Its tough to say at this point. Because were sitting tight and waiting and hoping. Okay. Here is a great question coming in on facebook. Does the virus strain of the donor need to match the donor strain of the recipient. Thats a great question. Its like youre dealing with one thing. Theres a lot of, the people who develop antibodies are successful at it. Those aenntibodies work against new virus. There isnt a lot of mutations that we know of so far. If somebody got it, those fighting cells would likely very much so work in the person who doesnt have them yet. Then you might have just answered this next question but maybe not so im going to ask. Does having recovered from one strain confer immunity against another strain . When people read the media, you can do very, very fine analysis. Its really one little fragment of amino acids. From the bodys perspective its the same thing. You might read newspaper that new york had more of europe. K it all is to the body its the same covid19. Thats why i think the vaccine would not be like super hard to do. Its not like the hiv vaccine where its mutating all the time. This is like one thing. The body sees it, scientists will develop that vaccine to this one thing and we can move on with life. Another question thats popped up in the last few days is that theres talk of strategic infection of sending young, Healthy People out there either into wa would be considered a sort of high risk situations or purposely infecting them so they could develop the antibodies and immunity and when theres enough people with immunity out there in society, the idea is you dont give it to each other anymore and you start to kill it and squash it. What do you think of that . Im frightened of that for several reasons. The idea is not knew. People have been doing chickenpox parties for a while. People nervous about getting vaccinations. First of all, theres no guarantee that the young person that youre putting out there to get covid wouldnt get super sick. In fact, the biggest age group in San Francisco county of people who got super sick and were hospitalized was a surprising age grown up, it was under age 40. Theres no guarantee that person wouldnt get sick. If they didnt get sick, they would easily infect grandma or grandpa who would potentially get super sick and die. Okay. All right. Thank you so much. Seconds. Were back with dr. Peter chin to answer more of your questions. Doctor, lets start with this from monica. Wearing your face mask any time doesnt matter that you have system, i think she means symptom or not, dont be child like. Okay. I think the face mask issue is one that a lot of people are still asking. Specifically, ive seen people asking about exercising. People find it hard to breathe and get their oxygen in if they are wearing mask. What do you think about that . I think its a tough thing. Even myself when i want to go running, i worry about that because its kind of difficult to breathe through a face mack masting. I would say probably the easiest thing and thats not easy for every one, i understand, is to choose a time of day when there arent a lot of people or choose a place where there arent a lot of people and that might be difficult to find but thats probably the best way to not wear the mask. Otherwise, you can imagine a team, a track, high school team, although theyre not supposed to be running in a group, runs by you. You kind of get caught unawares and not wearing a mask and thats kind of like a tough place to be at. Again, choosing, controlling your environment is key. Okay. Mona wants to know will there be a vaccine before schools open again . There wouldnt need to be vaccine before schools open again. Schools as we know it and some of the predictions about how schools will be once we open, they are very interesting. They will not be the schools that you recognize. Theres been talk about going back to school in shifts so they might be monday, wednesday, friday group and a texuesday, thursday, saturday group. You can have half the number of kids so you can maintain social distance in easier fashion. That will probably also extend to workplace or some workplaces where you can divide the gains and half the employees will go in onehalf and the other time will go the other time. Apart from doing tele work, et cetera. Heather is asking can you explain the shaking with chills versus just the chills again. The chills are like i go, im feeling cold and my body is trembling a bit. The rigors is like my bed is moving. Im going like that. Thats the difference. Its the same thing just amplified. Its really the same symptom. One is bit more severe than the other. None of those are very specific. They dont get included in my top two, which is fever and can i smell the coffee. Teenager with diabetes type one, stay away from grocery stores. A good question on behalf of folks with diabetes. Thats great question about diabetes. I think we worried a lot. Were still bit nervous about people with other medical problems. I would say that the teen with diabetes and diabetes is very common so you think about the risk of the situation. The teen with diabetes, again, does all the things that we as a society recommending meaning if you go to to grocery store, make sure you wear a mask, wash your hands a lot, maybe wipe down that iphone when you get home but main thing is protecting your nose and mouth, youll be fine. Its only if you didnt protect your nose and mouth and you went to that grocery store, that teen with diabetes would be at risk. If youre doing those things the, it will be pretty safe. All right. Heres a good question t coming in from phillip. I always get the flu shot this year. Should i get it early and will the vaccine for this one be different . Thats great question. Very sophisticated questions from the group today. I think that usually some people wait a bit later in the season for the shot. With the idea if the flu vaccine doesnt seem to be working, you can kind of get the later version of the vaccine. I think with covid19 upon us, i probably wouldnt wait because i want want to protect myself against something so i can empower my body to not get two things at the same time. The worries that you get covid plus flu which would be much worse than getting covid alone. All right. We should get our flu shots this year.
