Perhaps needlessly. The city might run out of ventilators by early next week. So the governor is going to commandeer them from places that dont need them right now. Im not going to let people die because we didnt redistribute ventilators. The National Guard will be deployed to pick up these ventilators, which are all across the state, and deploy them to places where we need them. Reporter the 1,000bed usns comfort docked on monday, but still there are only 20 patients on board. Red tape, were told. The navys position is they dont want to put covid people on the ship because it would be too hard to disinfect the ship afterwards. Thats my rough interpretation of what theyre saying. Reporter and those two cruise ships with sick and some dead on board, one of them finally allowed to dock in florida. The sick will stay on board for treatment. The walking well given masks and bused to the airport. There are now more than 1 million confirmed coronavirus cases worldwide, according to Johns Hopkins university. And nearly a quarter of them are here in the u. S. Where there is no national stayhome order. And some states are Still Holding out. If you look at whats going on in this country, i just dont understand why were not doing that. We really should be. Reporter 12 states also exempting religious as he wasnt from their stayhome orders. I dont think the government has the authority to close a church. Im certainly not going to do that. Reporter here is what can happen. Health officials tell cnn that 71 infections and one death are all connected to this one church in california. Its essential that we practice physical distancing everywhere, period. Reporter mayors of our two most populated cities now telling us to wear masks outside. Its under consideration at the federal level. The most important thing is to keep the sixfoot physical distance in individuals. This is an addendum and an addition to the physical separation, not as a substitute for it. No one really knows how long this is going to last, but here are some hints. Here in santa monica, closed stores have boarded up for the long haul. The mayor of washington, d. C. Says they may not see a peak in cases until late june early july. And at least ten states have now closed school for the rest of the school year so kids arent going to be going back until the end of august at the earliest, jake. All right, nick, thank you so much. Joining me now is cnns chief medical correspondent dr. Sanjay gupta. Sanjay, the Trump Administration is debating whether or not to issue new guidelines that americans should wear masks whenever they leave the house. If theyre debating this and if, you know, cities like new york and l. A. Are doing it, and dr. Fauci thinks we should be doing it, is there a Company Already out there producing them en masse . Im told its simple cloth, easy to make. Is haynes or fruit of the loom or any company on the case . I havent heard that yet. I mean, you know, these recommendations, obviously, jake, still havent officially come out. I am pretty sure that they are, and as you know, there are a couple of cities recommending it for their citizens. So far, and i talked to dr. Fauci about this, he says theres going to be a specific recommendation coming soon. In the meantime i think a lot of people are sort of making their own masks and theres a lot of doityourself instructions online. Jake, even my daughter, she heard us talking about it the other day and she made a mask for me and my family members to wear. Nice. Yeah, its nice, it works well. The idea that people were told not to wear masks and now, wear masks, its a little confusing. These are not masks, as you mentioned, that shouldnt be taken away from Health Care Workers, these are not n95 masks. These are masks to protect other people from you because we all might have the virus, we have to behave as if we have the virus, which weve been talking about for months now. Lets talk about that, because obviously the instructions changed. The Surgeon General was forceful in february saying, dont go out there and buy masks, masks wont help you. Was that, as ive heard it described, a noble lie, in the sense that they wanted to make sure Health Care Workers in the front lines got the masks and they werent taken or hoarded by general people in the public, or is it more benign than that, is it because it turned out, and we didnt know this at the time, Health Care Experts didnt know at the time that its much more easily transmissible than we thought, and for that reason the message changed . Yeah, i think its probably, you know, as with most of these things, a combination of both, but probably mob tre the latter. Certainly we dont want to take away masks from Health Care Workers. It was extraordinary, it was already anticipated back then, two months ago now, that we were going to run short, that despite everything we knew about this pandemic, that these masks would run short for Health Care Workers. What has changed, over the last few weeks, is the clear recognition that even if you dont have any symptoms, no coughing, no sneezing, whatever, that you could still be releasing the virus from your nose and your mouth. So anything you can do to decrease the amount of virus that youre putting into the environment is good when you go outside, when youre in public, when people are around you. Obviously the primary advice remains the same, stay home as much as you can. And dont use this to lull yourself into some sort of false sense of security. But a cloth mask couldnt probably decrease the amount of virus that youre releasing, so youre not really doing it to protect yourself, youre doing it to protect the people around you if and when you have to go into the public. Its a change, it will be confusing for people, but i think i get it now, i understand the science, like i said, my daughter made me this mask, i will probably use it if i go into public. You dont have to have a mask like this, you can use something else, but that is something were going to see in this country. I wanted to ask you yesterday, we ran out of time, you live in atlanta, and your governor, kemp, said yesterday, when he announced that he was finally issuing some guidelines and recommendations for your state, that he didnt know until the previous 24 hours that astoma asymptomatic people might be carriers of coronavirus. Youve been saying, and the cdc, which is also in georgia, have been saying that asymptomatic people are carriers, for weeks if not months. How is it that the governor of your state didnt know that . To be clear, its months, jake, you know, months, beginning of february. I went back and looked at this, its ridiculous, jake. My wife says shes seen me the angriest on your program, once several weeks ago when we werent seeing social distancing despite the fact that it was clear that it was necessary, and now talking about the governor. Its unbelievable to me, just a flatout excuse, it makes no sense. He was completely not paying attention to the Biggest Issue of our time, possibly, you know, increasing the likelihood that citizens of his state would get sick, even die, possibly that it would increase the likelihood that hospitals would become more overwhelmed, and he was seeing some of the hospitals in georgia actually starting to become overwhelmed, in albany, for example, georgia. I dont get it. I dont understand it at all. Maybe theres things at play that are just mysterious to me. But he said yesterday that he just found out over the last 24 hours that asymptomatic spread was possible, that people who didnt have symptoms could spread that. The cdc, which is right down the street, has said that. My kids who go to grade school in this state already knew that. I mean, everyone knew this. So its really ridiculous. Im glad that theyre at least going to start the stayathome order, i believe tonight. Its still not in police, i dont believe. But one day, jake, you and i can go back and look at what exactly happened there. Its some of the most confounding things ive heard and seen from leaders, ranging from the president to your governor to other governors, people not paying attention anyway, let me move on to another question. Sanjay, we keep hearing about desperate cases among not older people, younger people. Weve seen this time after time after time. Its not just people who are 60 and over or people who have preexisting conditions as was the original guidance. It might adversely impact older people but it certainly does not spare young people. Do we have any idea whats going on or any idea of the percentages now in the United States in terms of how often it is children, young adults, you know, middleaged people are getting this as opposed to seniors . Yeah, well, you know, look, its very clear, and i can show you the numbers, the percentages right away here. We look at younger people, people clearly below the age of 60, and we see, you know, that if you start to do the math there, the vast majority of people infected are actually 65 and younger. And you will note that 20 to 30 of people who are in hospitals, who are hospitalized with this infection are between the ages of 20 to 44. This clearly can infect younger people. They are not immune to this. It can clearly infect them and make them sick. Sometimes the impact can last a long time. Part of it is probably because theres a lot of preexisting conditions even among younger people. I mean, you know, older people obviously are more vulnerable, but younger people can be affected by this. Ill tell you quickly, jake, older people are likely the virus is sort of maybe overwhelming their immune system and thats part of the reason theyre getting so sick. With younger people it could be their immune system reacts to strongly to the virus that they develop whats called a storm, a cytokine storm in their body, and its actually the inflammation that is making them so sick, and sometimes in rare cases actually cause death. First, he and the administration is doing an excellent job and any complaints are fake news. Second, the ventilator shortage issue is the fault of the governors and the hospitals, not his fault. Even the lack of working ventilators in the National Stockpile is not his fault. As Cnns Kaitlan Collins reports, governors arent the only people trump is pointing the finger of blame at. Reporter the white house is expected to make a final decision on whether all americans should wear masks in the coming days. Though some of president trumps advisers fear it will lull the public into a false sense of security. We dont want people to feel like, oh, im wearing a mask, im protected, and im protecting others. Reporter while trump and his aides decide on guidance, a new face emerged yesterday in the administrations response to the coronavirus. Let me introduce Jared Kushner. Reporter in his first briefing on the pandemic, Jared Kushner drew backlash for this remark. The notion of the federal stockpile is its supposed to be our stockpile, not states stockpiles. Reporter but many pointed to the National Stockpiles website which said it was for state, local, tribal, and territorial responders who need federal assistance. That language was changed overnight to fit kushners description. The move added to the mounting frustration among governors who are looking to the white house for help to get ventilators, masks, and other lifesaving equipment. This will go down in history as a profound failure of our national government. Reporter the president is also now feuding with a company he once said was proof of his success. Vice president pence toured 3ms headquarters in minnesota last month. But now trump is using the defense production act to force the company to make more masks. And 3m is pushing back. The narrative that we are not doing everything we can to maximize delivery of respirators in our home country, nothing could be further from the truth. Reporter the companys ceo says trump wants them to stop exporting masks to canada and latin america, which the white house has denied. Were more than happy to ship our overseas production to the u. S. But there are going to be consequences on a humanitarian level as we are the sole, often the sole provider of those respirators in countries around the world. Reporter 3ms ceo is also warning that the move could backfire by causing other governments to clamp down on the export of materials to the u. S. Now, jake, republican Senator Cory Gardner has sent a letter to the department of health and Human Services requesting a probe into whats going on with the distribution of these ventilators. And when politico asked him about kushners comment about the stockpile remember said he didnt know what the president s soninlaw and Senior Adviser was talking about. He said the stockpile is for the country and of course the country is made up of those states. It seems to follow. Kaitlan collins, thanks so much. In money today, a chaotic start as 349 billion worth of essentially free money went up for grabs to an estimated 30 million Small Businesses in the United States. Final guidance to lenders went out late last night, only hours before banks were set to begin taking applications. I want to bring in cnn business anchor julia chatterley along with brock blake, ceo of lendia, which helps Small Businesses find loan options. Thanks to both of you for choing joining us. Julia, some lenders had concerns about taking applications. This was a massive undertaking, jake, but the start was chaotic. A formal Application Form was not confirmed until last month. Try building a website based on a form that you dont yet have finalized. Then there were other things. We can pay out billions of dollars, that was the message from the banks, but whos going to replace that money to allow us to keep making loans, and basic things like who carries the risk of verifying the borrower information . The treasury final said last night, look, just make the loans, well take on the risk here. But a lot of information came incredibly late, and the chaos today represents and reflects that. And brock, you lit up twitter with questions from lenders, 24 hours before the lenders were supposed to start taking these loan applications. Were most, any, some of the questions answered when the Treasury Department finally released the guidance last night . Well, some of the questions were definitely answered last night, mostly being what form should the lenders release . They scrambled last night to make sure businesses could go online and apply for these loans. White house economic adviser larry kudlow said today, quote, there are always glitches. Are the glitches understandable given a program of this magnitude rolled out in just seven days while companies are shedding jobs daily or was there some incompetence here . Theres no question, this is the largest stimulus package in the u. S. They had days to make it happen, aligning Lending Institutions and Business Owners. Unfortunately the guidance came late. But the fact that dollars are flowing today, to me is a big one. Most Lending Institutions are not prepared today to begin lending. I think that some of them will come online this afternoon. Probably more likely the majority of Lending Institutions will come online on monday. The challenge is, most small banks and Credit Unions arent prepared to spin up a technology to be able to accept applications so quickly. Were doing our best to help all of them make that happen. But its an overwhelming undertaking in such a short amount of time. Julia, before noon today the Treasury Department reported nearly 900 million worth of loans already processed. Small businesses of course make up more than 80 of americas workforce. That really speaks to just how many of these Companies Need this money. But that not really even a fraction of the money thats available. Absolutely. I mean, the treasury secretary tweeted earlier today that actually 1. 8 billion worth of loans had been processed. But your point is exactly right. And we know the situation across the country right now. 30 million businesses struggling. Bank of america said halfway through the day today they had had 58,000 customers asking for 6 billion. And they restricted it to clients, remember. So its a beginning. Its been quite chaotic. I think this big question is what now needs to be fixed to ramp this up more broadly. Brock go ahead, go ahead. I was going to say, i think theres two solutions that will help get capital into the hands of Business Owners more quickly. One is, they have to allow fintech lenders to participate in the distribution of thats loans. They have the technology available. Theyre set up to handle high volume. And they can participate. In addition to the Community Banks and the large and credit card unions that are participating. The other thing is that most Financial Institutions dont have the Balance Sheet or the capital to fund the demand of loans out there. So if the treasury can give some guidance on how a Lending Institution can fund the loans and then get reimbursed or replenish their capital so they can keep funding the loans to the Business Owners that are out there, there is so much demand. A lot of these Financial Institutions are going to go through their capital way too quickly. I agree. All right, brock blake and julia chatterley, thanks to both of you, appreciate it. Medical professionals tell cnn they hope it never comes to it but theyve developed a point system in the inevitability hospitals need to decide who gets lifesaving care and who dont. Its not inevitable but it might happen. Drew griffin has more. Reporter the horrifying pictures from inside new York Hospitals show patients hooked up to the only machines keeping them alive. When the virus takes over the lungs, ventilators take over the breathing. Without them immediately when needed, the prognosis is dim. You can watch a patient go from breathing room air to 72 hours later, needing to be intubated. Reporter so far hospitals have kept pace, but barely. The situation so bleak, the u. S. Government put out a video on treating two patients with one ventilator. You wouldnt do it unless you were in dire circumstances. Reporter but the dire circumstances are here. States are in a bidding war for ventilators made in china. We cant get any more ventilators. Reporter it may be time to prepare for the inevitable. This article in the new england journal of medicine was written to help doctors in the event they must choose who gets a lifesaving ventilator and who does not. This man is one of the authors. Many hospitals have these in place. I think its going to be extremely difficult. Unbelievably difficult for those physicians who have to make the call. Thats right. And of course the families and patience as we patients as well. These are life and death decisions. Its going to cause a tremendous amount of suffering if we get there. Reporter the decision, who lives, who dies, would come down to a point system. The elderly, patients diagnosed with cancer, copd, diabetes, any chronic lung or terminal illness, would be eligible for care but score lower than those who are otherwise healthy with a potential longer life to live. The points would determine what is in the best interests of society, not just the individual. Everybody is eligible. But beyond that point, then it does come down to giving it to those people where were either going to save the most number of lives or the most number of life years. And yes, it does mean that people with other severe illnesses will receive a lower priority score. Reporter in new orleans, where the virus is predicted to get even worse, there are enough ventilators now. But within days they could be out. And then after that you begin having very challenging conversations about how you allocate the vents and you think about which patient would benefit the most. Thats a horrifying for anyone to be in, and a damning indictment for our country. Reporter jake, what is infuriating about all of this was, it was predicted. Cnn has found ten government reports between 2003 and 2015, all predicting that if we face a pandemic just like this, we would run out of ventilators. And here we are, jake. Drew griffin, thank you so much. One New York Hospital has found a creative way to convert other medical equipment into ventilators by using basic parts they already have. Joining us for an exclusive interview to explain how all of this works is dr. Charles powell. Hes the chief of mt. Sinais pulmonary Critical Care and sleep division. Dr. Powell, thanks so much for joining us. I understand youre using simple parts to help convert cpap machines into ventilators. Explain to us what a cpap machine is, how you came up with this, and show us how it works. Sure, thanks very much, jake. So we were fortunate to receive a generous donation of these home cpap machines that are used for patients with sleep apnea. The donation was from the tesla company. And so the question came, can we use these simple devices to ventilate a patient in one of our icus who has complications from covid pneumonia. We have a very experienced and Accomplished Team of sleep physiologists here at mt. Sinai, and they said yes. We said, how . They started to explain it, and it was really complicated. We just said, it needs to do x, y, and z, in terms of ventilating a patient and it also needs to avoid spreading the virus. They constructed a protocol and a simple circuit. So when you take that Little Machine i showed you and hook it up to a test lung, it inflates the lung and does it in a way so the virus doesnt get moved around and it doesnt get exposed to anybody in the area. So we took that to the simulation lab, we tested it, improved upon it. Its been evaluated in a clinical setting and it works. We now have received subsequent donations of similar machines and well be able to increase our ventilator supply to put us in a position where we can be in very good shape to meet the anticipated surge of patients who are going to need ventilators. So as you say, medical students are now assembling these machines for hospitals to be able to use. How many ventilators have you been able to use . Is this simple enough for hospitals across the country just to follow basic directions and make them themselves . Yes, the protocol is simple and has already been released widely. It is available for anyone at any time. So with the appropriate supplies in hand, and i must say that there is a shortage of supplies in some locations, similar to the shortages of many supplies throughout the country, but with the appropriate supplies in hand, any hospital can do it, following the protocol. They dont have to use the same exact machine. They dont have to use the same exact circuit, just something similar that incorporates the filters that are incredibly important to prevent the spread of the virus. Then that could be used to increase the supply, using these very inexpensive, very available machines. They dont work on every patient who needs it who has covidassociated complication. Patients early on need a more conventional ventilator that is more powerful. But when patients get better, they can be supported with a simpler device just like this. So youre also trying to cut down on the amount of protective gear that doctors and nurses need. So you also made these machines controllable from outside the patients room if need be. Yes. So i just want to clarify, this machine does not reduce the need to wear the appropriate protective equipment when were in the room. But what it does do is avoid the number of times somebody may need to enter the room and be exposed with our ability to monitor and control the device from outside the room. And youre working in the epicenter of the coronavirus pandemic in the u. S. , new york. Can you explain to us just how bad it is on the front lines, at mt. Sinai, how much worse you expect it to get . Its bad. It was the kind of event that maybe had been predicted but never was thought to be able to occur. And its occurred. All the hospitals in the area have developed strategic plans. And those were developed months ago. And theyve been in action for several weeks now. And the goal of all of these strategic plans is to stay one step ahead, one step ahead, making sure we had the adequate personnel and equipment to take care of the surge of patients who are going to come to our hospitals. So the hospitals are overwhelmed in terms of volume. But by overwhelmed, i mean it just takes initiatives to create additional bed space, to bring in additional staff, to bring in additional supplies, to put us in a manageable position to handle the surge, which weve been able to to do. But really, the main problem is volume. This is a volume crisis in terms of the number of patients who need hospitalization for covid19. And the primary determinant of the volume is the doubling time. And that is affected directly by social distancing. So what weve seen, to address your question of how long this is going to last, is that over this past several weeks, the curve has flattened in new york in particular. Each day, the peak number of patients requiring hospitalization seems to decrease by 1 to 2,000, maybe a little bit more some days. So at peak, we think, if social distancing continues to be implemented and is effective, is going to make the peak at an area where this will continue to be manageable, although the peak will go on longer, with the peak anticipated in mid to late april. Perhaps as shocking, the European Unions tepid response, not even mentioning hungary by name. Any emergency measures must be limited to what is necessary and strictly proportionate. In the uk, one Regional Force was criticized for using a drone to film a couple driving to a beauty spot to walk their dog, then shaming them by posting it online. How much freedom to give up, for how long, and under whose control. What orban is doing in hungary goes way beyond that debate. Europes most liberal democracy just lurched toward a russiastyle government. Orban has been refusing to take in migrants during the 2015 crisis, more recently hollowing out hungarys judiciary, turning the country into a oneparty state. All this when truth about the coronavirus pandemic is at a premium. Now it is more important than ever that journalists are able to do their job freely and precisely so as to counter disinformation and to ensure that our citizens have access to crucial information.
