A groundbreaking pilot program, Family Bridge, spearheaded by Connecticut’s health and human services agencies, has provided vital assistance to over 500 area families with newborns since its launch last October. Offering expert at-home nurse and community health worker support, this initiative aims to smooth the transition for parents from hospital to home, offering crucial guidance and care in the crucial days and weeks post-birth. With no eligibility criteria, it’s funded through various federal and state resources. Services provided include health screenings, education, and connections to essential resources.
