Oregon State Hospital plans to train medical staff to immediately check the vital signs of new patients and respond to life-threatening emergencies with adequate equipment.

Staff at Oregon State Hospital failed to make hourly checks of a patient who died of a suspected fentanyl overdose in May, public records and hospital staff said.

It's the latest in a string of deficiencies investigators have found at the state psychiatric hospital.
