Deputy Health and Disability Commissioner Rose Wall today released a report finding rest home owners Oceania Care Company Limited in breach of the Code of Health and Disability Services Consumers’ Rights (the Code) for failures in its care of a man who suffered two falls.
The man had a medical history of poor balance, severe fatigue, impaired hearing and end-stage renal failure. At the time of events the man had been a resident at the rest home for only five days.

Disability Services Consumers’ Rights (the Code) for
failures in its care of a man who suffered two
falls.
The man had a medical history of poor balance,
severe fatigue, impaired hearing and end-stage renal
failure. At the time of events the man had been a resident
at the rest home for only five days.
The first fall
occurred when a staff member helped the man to the toilet,
and left him alone. The second fall occurred when the man

today released a report finding a locum GP in breach of the
Code of Health and Disability Services Consumers’ Rights
(the Code) for undertaking an inadequate assessment of a man
in her care.
The man, in his sixties, had a history of
diabetes, hypertension, and heart disease. He saw a locum GP
because he’d been having indigestion and a lack of
appetite for several months, had a fever and was
exhausted.
The locum GP discussed with the man his
symptoms, recent medical notes and medications, and
diagnosed him with gastritis. She prescribed medication and

released a report finding a registered nurse in breach of
the Code of Health and Disability Services Consumers’
Rights (the Code) for failure in her care of a baby being
vaccinated.
At the time of these events, the baby was
overdue for his six-week vaccinations. During a home visit,
the nurse administered the wrong six-week vaccine to the
baby.
Instead of the rotavirus vaccine, the nurse
vaccinated the baby with Prevenar 13, a vaccine that is
normally administered by injection for protection against
pneumococcal disease. This was in addition to the PCV10
