The service aims to support the diagnosis and treatment of people with heart failure living in Donegal.
It has been developed as an integrated team to support earlier access to diagnosis and treatment.
The new service allows GPs to directly refer patients they suspect of having heart failure to the community service.
It also allows patients who have been admitted to Letterkenny University Hospital with heart failure to have their follow up in community locations reducing the need to travel to Letterkenny for many.

Latest figures, show that there are 4 cases at the hospital.
31 Covid patients are intensive care units nationally today - the lowest this year.
The figure has fallen sharply from 221 in late January.
124 Covid patients are in hospital today - an increase of eight from 24 hours ago.
Meanwhile, there were 25 people awaiting admission at Letterkenny University Hospital this morning, the third highest figure in the country.
According to the INMO 7 of the patients were waiting on a bed in the hospital s Emergency Department.

Tributes have been paid to a County Derry man who has passed away after falling into a river during a fishing trip in Donegal.
Michael Conway, from Draperstown, is thought to have been fishing between Glenmore and Cloughan in Ballybofey when he fell into the river.
Emergency services recovered his body from the river after attending the scene, with a post mortem due to take place at Letterkenny University Hospital.
Local sports clubs Draperstown Celtic FC and Ballinascreen GAC led online tributes to Michael following news of his death.

Gardaí and emergency services attended a house on the Donegal Road following the discovery of the body this afternoon.
Gardai have confirmed that the woman s body has been removed to Letterkenny University Hospital.
