By Todd Epp Fatal accident graphic. SIOUX FALLS, S.D. (KELO.com) – A Sioux Falls man has been identified as the person who died Saturday morning in a two-vehicle crash west of Sioux Falls. A 2000 Lexus SUV was southbound on LaMesa Drive and failed to stop at the intersection of South Dakota Highway 38. It collided with a 2013 Honda Pilot that was eastbound on South Dakota Highway 38. Both vehicles came to rest in the south ditch. Alonso Chavez, the 34-year-old driver of the Lexus SUV, was not wearing a seat belt and was pronounced dead at the scene. Kelly Carlson of Hartford, S.D., the 52-year-old driver of the Honda Pilot, was wearing a seat belt and suffered minor injuries. She was taken to a Sioux Falls hospital by private vehicle.
