Senior reporter steven stock has been on this case for six months. Steven. Reporter this independent forensic report just released says that the state failed to use modern engineering standards to address issues with the spillway that failed back in february. But our investigation uncovered warning after warning that the dam itself needs serious attention and repair. Problems that two former state insiders tell us are being kicked down the road and not being fully addressed now. All summer and into the fall work on orville collapsed spillway has forged on at a furious pace. As

Water Resources<\/a> races the clock to repair the mile long spillway before the next rainy season. But whats not getting all the attention that concerns several engineers we spoke to. They dont know what to do. Reporter state
Water Resources<\/a> retired after working on many facilities including orville dam. I think it has failed. It just hasnt collapsed. Reporter we examined two decades worth of federal and state inspection reports, engineering studies and memos, nearly 10,000 documents in all. They repeatedly identify safety issues which concerned several engineers we spoke with. Issues such as 15foot long crack in the concrete for gate 8 that appears to be growing. Spalling or deterioration of concrete in other areas. Cracking trunnion rods that help move the gates that control the flow of water through the dam. And failure to come up with a longterm plan acceptable to inspectors that would monitor the flow of water that goes through the eitharthen dam. If that comes out at the wrong speed it could wash the dam away. My concern is for the public. Reporter this recent high level eveninger also worked on the project. And not addressing issues that have been race raised and pointed out in previous inspection reports. Reporter mark joined another retired dwr engineer, tony, who worked on state
Water Project<\/a>s including orville dam. Both want today share their knowledge from the inside about how dwr runs, manages and maintains its dams. They have a tendency to try and reduce maintenance costs by trying to do things themselves and not getting adequate technical help. Reporter they asked us to conceal their identities because both fear their pensions could be taken away from speaking out. Youll have a catastrophic failure thats not going to allow you to operate the facility the way its supposed to. Were pretty confident that, you know, the dam is safe. Reporter joel serves as

Structural Engineers<\/a> looking at the cracks, so, today, yes. Obviously we need to keep monitoring. We need to improve our maintenance program. Reporter critics such as engineers who used to work there, says the issues must be addressed now. Issues like that crack which this report shows in red ink and says is growing in length. Cracks like that could weaken the structure to the point where it could lead to a failure. Reporter well, i think thats probably a miscommunication. Ive had conversations with our dam safety engineers and it has not grown. Reporter cracks in the rods that control the
Flood Control<\/a> gates worry engineers we spoke to even more. Initial tests says 28 of the rods have cracks. Cracks are larger than 116th of an inch. They say more comprehensive tests are called for. Back when we were looking at it, these bolts were cracking and failing. If this fails you wont be able to control the releases out of orville. Reporter when i show you that, what does that say to you as an engineer . It meals me we have a problem. And it tells me that we are lucky that it hasnt manifested itself to a great problem in a crisis. Reporter but what draws the largest critical questions from the half dozen engineers we spoke to was this. Dwr has failed to dwell, quote, a plan for longterm monitoring of water that naturally leaks through the dam. Whats known as the surface. Inspectors raise this concern repeatedly in reports over years. Listed as top one to address. They say dwr must know with certainty how much water is coming out of the dam and where. Because if water flows out of the dam face instead of the toe, erosion can occur and the entire dam could wash away. Reporter clearly federal monitors are concerned about this issue. We have a monitoring today. Reporter but forgive me the fact they keep pointing it out and repeating means it hasnt been addressed yet. Whats taking so long to address these issues . That i cant answer. Thats something i can get back to you on on why so long. I dont have an answer for that. Reporter dwr never did get become to tus on that question about why all this is taking so long. Also would not let us see or take our own pictures up close at gate 8. And researchers have been trying to figure out a better way to test those helmets to prevent those concussions. Now they say they finally figured it out. Heres nbc jeff. Reporter across the country kids back on the football field. On the sidelines, anxious parents. According to safe kids, over 160,000 kids and teens went to the er with a sports related concussion in just one year. Thats one concussion every three minutes. Football, the leading cause. For decades this is the way they tested helmets to make sure they are safe. But starting now, a brand new tech, and experts say its a game changer. We are at this special helmet lab where they are testing helmets from the nfl and also youth helmets. The kind of helmets your kids where on the field. To meet the new standard here in the lab. Lets check the first test. This is going to come up right and dropdown, that has been used by all of the years. Explain whats happening here. Right sochlt this is the linear drop test. And what it does is shows how theyll protect the head from skull fracture. But you are saying this is not sufficient. Its not testing ahead turning but hit like this. Yeah, used for decades but a little one dimensional. Okay. This is coming down so well watch it. Thats a big hit. Take a look in slow motion. The head barely moving. Doctors say it doesnt simulate a real football hit with a neck twist. If you take a blow to the side of the head, especially far forward or backward, you are going to spin the head on the neck, like that. Its the type of injuries that lead to concussions. Whats different about this test it incorporates a neck and allows the head to move in ways that are realistic to the way it would on afield. Reporter and you have this set to the youth level.
