WEST HAVEN — The U.S. Department of Veterans Affairs Connecticut Healthcare System received nine notices of unsafe and unhealthful working conditions following a U.S. Department of Labor Occupational Safety and Hazard Administration inspection after two maintenance workers died in a steam explosion in November.
