Reporter the ride went wrong when the operator couldnt get the train to start and asked to go into what the mbta calls bypass mode. The operator exited the train to undertake that procedure and while the operator was not on board the train the train left the station. Reporter because the train was in bypass mode the brakes on the train appear to have not been engaged and the trains collision system was disengaged. Theres a twolayer system so that this doesnt happen, says the mbta. The first layer of protection are the procedures and requirements that are supposed to be followed and the second layer is ensuring that our those procedures. Reporter fox 25 exclusively obtained this photo from inside the cabin. Passengers tell fox 25 that a cord was wrapped around what they believe was the accelerator of the train. It would be a prohibited act if in fact there was any type of cord that was wrapped in that manner, so that will be part of the focus the investigation. Reporter . Investigation continues we wanted to learn more about the mindset of a conductor. Transportation consultant john tells us standard procedures for conductors to get in the cab, turn the throttle from off to on and stay there. Typically they would radio to the tower that he was leaving the cab. Reporter reviewing this photo, he also found it unusual to see the key in the door. I would suspect that the cab would be active if the key were in the door. If the train is out of control, a conductor can remove the train but in this case with no conductor on board that couldnt happen and the mbta instead had to cut power to the third rail. Thats usually the last resort, is deenergizing the third rail to remove the power so that the train couldnt move. The train could only move with the third rail energized. Rarely have i ever seen a conductor leave the cab of a transit train without locking the door behind them. Unlike airplanes where theres a pilot and copilot, justice tip says its common for trains to only have one conductor because there are several safety mechanisms to stop the train in place.
