Officers involved in the Sept. 3 shooting of a Portland activist described a tense atmosphere and a chaotic scene caused by communication problems, according to investigation documents newly released April 6. In reports written after the shooting, the officers said they were warned by their superiors that Michael Forest Reinoehl, 48, was armed, and a friend allegedly had told police Reinoehl would not be taken alive.
