Here s dr. jon lapook. reporter: the food and drug administration tested 50 vials suspected to be a source of contamination in the meningitis outbreak. they found all 50 were contaminated by bacteria or fungus. the f.d.a. says internal n.e.c.c. documents show the company tested a vial from the same lot in august and labeled it sterile. the inspection also focused on the clean room at n.e.c.c. a clean room like this one is where ingredients are mixed to make sterile medication. the f.d.a. discovered n.e.c.c. s own monitoring repeatedly found mold and bacteria growing on walls and surfaces. equipment used to sterilize medicine had greenish-yellow discoloration and air ducts were dirty. according to the f.d.a., there s no evidence n.e.c.c. did anything to fix the problems. the n.e.c.c. responded today saying we will review this report and will continue our cooperation. ....

Ginger zee now. reporter: breaking in, spraying down. tonight our nation is in assault mode. the mission eliminate mosquitoes that could carry the west nile virus. in care carolton, texas, they are busting into abandoned pools and cleaning them out. that way, mosquitoes can t breed in stagnant water. in new mexico, north carolina and illinois, it s a spray that they hope will protect. went to bed that night, and and next morning, i woke up, i was blind. reporter: that s hal dalton, oklahoma s first confirmed human case this year, he is one of the nation s almost 700 infected with the virus. with at least 26 people dead, this is the worst outbreak in the west nile virus this early in the season, ever. ground zero this year, dallas, texas. there s no question that there s something different going on in texas this year. we would not call this a major epidemic. reporter: so, no need to panic says west nile virus ....

8-year-old, and in good health. at least she was until a mosquito bit her last month in her backyard outside dallas. just about the time i think i m out of the woods, something else comes along and we have to fight that, too. reporter: you have felt miserable for more than two weeks. i have. reporter: that one mosquito bite has left her first with a fever of 104, and now, with meningitis and encephalitis. dallas is the epicenter of this epidemic. one quarter of the country s cases are in this one county. most people with the virus never know they have. roughly 80% have no symptoms. 20% suffer from some combination of high fever, headaches, joint pains, vomiting and diarrhea. only about 1% develop the neuro invasive form of the disease that attacks the brain and spinal cord and can be fatal. we re starting to see some younger people get neuro invasive disease, and that s a little bit unusual. ....

reporter: communities across north texas have been spraying pesticides every night. and by the end of the week, some will start controversial aerial spraying. i don t want anybody to get this. so, if that means they have to go and spray from the air, do it. reporter: doctors say kathryn will recover, but she s already missed her daughter s first day of school, after a summer few here will forget. ryan owens, abc news, dallas. and i want to bring in abc s medical editor, dr. richard besser, spent much of his career tracking diseases with the cdc. so, 400 people have already had the most severe consequences. what are we going to do about this? reporter: yeah, i mean, it s very serious. and there is no treatment. so, the key here really is all about prevention. when you talk about prevention, the key is, fight the bite. if you don t get bit by a mosquito, you re not going to get this disease. the woman that we saw in this story, it doesn t always go away. 60% of people continue ....
