Virginia state law requires you to provide us with a health history and an official immunization record. William & Mary also requires you to provide documentation of the same immunization requirement. This information must be submitted through the Online Health Portal.
When fulfilling this requirement:
Keep in mind, you must complete the immunization form by BOTH (1) entering the necessary data AND (2) uploading official immunization records. This necessary document will be returned to you for re-submission if both steps are not followed. 
While failure to complete this requirement can result in serious consequences, we understand that submission may be delayed due to current circumstances. Please complete the forms as soon as safely possible. You will be assigned a late fee of $100 if not returned by August 31. A hold may also be placed on your Banner account. ....
