老刘家住安徽农村，今年50岁。这个月月初，下地干农活回来，老刘冲了个澡，双耳不慎进了水，想着过一会水自己能流出，就径自开始吃晚饭，还喝了点小酒。坏就坏在老刘有多年的中耳炎病史，耳朵进水加上又喝了酒，第二天早上起来，他发现中耳炎复发了，双侧耳朵开始流脓。
口服抗生素没有效果用庆大霉素滴耳朵
滴了几次，耳朵确实不流脓了，可两三天之后，老刘突然觉得自己整个人头晕得厉害，同时还伴有严重的耳鸣，甚至连听力都严重下降，几乎听不到了。赶到县里的医院就诊，当地医生给老刘进行了抗晕眩等对症治疗，住院住了10多天，却没有明显好转，越发着急的他在家人的陪伴下来到杭州市中医院耳鼻喉科寻求帮助。
由于老刘的双耳听力已经严重下降，他的接诊医生耳鼻喉科主任陈志凌主任医师连写带比划，费了老大的劲才了解到既往病史、发病过程等基本情况。陈志凌告诉老刘，目前的情况很有可能是由庆大霉素导致的。
庆大霉素通过穿孔的鼓膜进入内耳
损伤耳蜗及前庭毛细胞
“当时我这么说的时候，患者还很奇怪，这种药不是消炎的吗？实际上，庆大霉素是一种抗生素，主要用于治疗细菌感染，确实有一定的消炎作用没错，但它也是一种耳毒性药物，在注射后常见的不良反应包括急性听力下降、眩晕、耳鸣等。”陈志凌说，原本只是往耳朵里滴几次庆大霉素，一般也不会有这么严重的症状出现，但结合检查发现，老刘的双耳鼓膜都存在陈旧性穿孔的情况。庆大霉素通过穿孔部位进入中耳及内耳，损伤了他的耳蜗和前庭毛细胞。
当耳蜗和前庭毛细胞损伤后，听力和平衡能力就会受到影响，临床上则表现为急性的听力下降、耳鸣、晕眩，和老刘听力及前庭功能检查结果提示的双耳重度感应神经性耳聋、双侧前庭功能减退相吻合。
还好CT检查提示老刘的中耳不存在较严重的病变情况，不需要进行手术治疗，陈志凌根据他的情况开具了相应的中药和西药。“一般来说，耳蜗和前庭毛细胞的损伤是不可逆的，而且，治疗效果和是否及时就诊也有很大关系，患者从发病到现在也有半个多月的时间了，听力是否能够恢复到从前目前还不好说，只希望尽力治疗可以带来较好的结果。”
每年七八月是中耳炎的高发季
