The heads of four City of Joburg (CoJ) entities must be hauled before a disciplinary committee for contravening by-laws because had they performed their duties, the Usindiso building tragedy that claimed 76 lives could have been avoided, Justice Sisi Khampepe has recommended.
The Usindiso building in Johannesburg where 76 people lost their lives in a fire was never zoned for residential purposes and at the time of the deadly blaze, it had a high prevalence of crime.
While the cause of the fire was identified as arson, the commission of inquiry found the poor state of the premises led to a conflagration that spread and destroyed the entire building.
