I think its perhaps a minority, and the second half was a lot better, so perhaps a victory all round. There were intense discussions between england players, management and officials through the first half, and an announcement was made to the crowd that the referee might suspend the game if the abuse continued. It was greeted by boos. There were nazi salutes on the ground. When englands players left this pitch at half time, they then discussed whether they should even carry on with the match. They decided to play on, and englands captain told me he believes that was the right decision. Everyone wanted to carry on and do their talking on the pitch, which im extremely proud of. Its not easy to play in circumstances like that. But the 6 0 fits, really, and the way we played, the manner in which we played, im extremely proud of, for sure. One answer to the abuse was the scoreline. Manager Gareth Southgate has openly acknowledged that english football has its own issues to deal with, but racism was displayed in its starkest, most blatant form here in bulgaria. More on all those stories in the next 20 minutes. A new International Study suggests that a cheap and widely available drug could save hundreds of thousands of people from traumatic brain injuries. The Research Suggests tranexamic acid, or txa, can significantly improve some patients chances of survival. Almost 70 Million People suffer these kinds of injuries every year, usually as a result of Car Accidents and falls. Our Global Health correspondent Tulip Mazumdar reports. In a brief moment, a head injury can change a persons life forever. Five years ago, pam foley fell off her bike in oxford. All i remember is one minute on my bike, next minute, on the ground, trying to get up. I knew i was stunned but i didnt realise how. How much i had hurt myself. Pam had fractured her skull and suffered a bleed on her brain. She did make a good recovery, but lost her sense of smell and taste. Its a constant reminder of the pleasures that can be had from simple smells. I really miss the smell of freshly cut grass, i loved that smell. This is a ct scan of a patients brain. This is unfortunately a young man who was punched and fell to the ground. There are currently very few Treatment Options for patients whove suffered these types of injuries, particularly in low and middle income countries. Patients can need surgery to ease pressure on the brain, or life support equipment. Researchers, though, say now theres another option, a simple injection of tranexamic acid. When patients are bleeding into their brain, naturally two processes occur simultaneously. The process of clotting and of breaking down the clot. Tranexamic acid stops the breakdown of the clot, so allows the clot to form more effectively, and hence reduces and stops the bleeding. This is tranexamic acid. Its been around for decades and was originally used for things like heavy periods. Its cheap, it costs just a few dollars per vial, and its already easily accessible all around the world. The Royal London Hospital here in the uk was one of 175 hospitals across 29 countries and involving more than 12,000 patients to take part in the trial. It found deaths in patients with mild to moderate brain injuries were cut by a fifth if the drug was given within three hours. Its a simple injection, it can be given around the world by doctors and nurses. There is no special training needed to give it. Previous International Studies have already shown that the drug can be used to treat women with excessive bleeding after childbirth, as well as patients with life threatening chest or abdomen injuries. Pam doesnt know if she received tranexamic acid or the placebo dummy injection in this trial, but she says shes glad to be part the study. The World Health Organization says it will evaluate the findings and consider whether to now recommend the drug for brain injuries. Tulip mazumdar, bbc news, oxfordshire. Lets speak now to professor Haleema Shakur still, who is the co lead investigator on this study from the London School of hygiene tropical medicine. We saw her in that report. You are very welcome. We know this drug has been used to deal with life threatening bleeding in the abdomen after car crashes and stabbings, for example, at what point did you think, lets see how this might work in the brain . During the time we were giving the trial looking at china exa m giving the trial looking at china exam acid for injury to the body, doctors were already asking, why arent you looking at it for injuries to the brain. At the time we were looking at tranexamic acid. We were looking at body injuries, we saw there was a potential to reduce bleeding into the brain but the trial was much too small to give it enough information on how to treat patients, but it offered hope. So based on the small trial we decided to set up this big trial we decided to set up this big trial to look at the bigger population, it is absolutely fantastic news. 175 hospitals involved across 29 countries, where you hopeful all still surprised by the results that you got . |j you hopeful all still surprised by the results that you got . I think we we re very the results that you got . I think we were very hopeful, because based on the evidence already available on tranexamic acid, introduces death from breeding and postpartum haemorrhage, introduces death from bleeding if you have injuries to your chest or abdomen it reduces death from bleeding in postpartum haemorrhage. There is a lot of information out there, the reason we had to do this trail is that currently most guidelines exclude tranexamic acid as a treatment for traumatic brain injury, so if you are ina traumatic brain injury, so if you are in a car crash and she had injuries to your head and your body, doctors were caught between giving you tranexamic acid or not, because if they gave you tranexamic acid for your head injury, it was a contraindication. So now we have even more evidence to support the evidence already the, so i would hope no doctors will go ahead and use tranexamic acid. I will come back to that in a second, but there isa back to that in a second, but there is a very specific direction for the use of this, to be effective, it does not work in all patients, but it saves the lives of around 20 of patients who have suffered traumatic brain injuries, i understand, in the trial, what time period do they need to get the drug in for it to be effective . You have to get it out sooner effective . You have to get it out sooner reading starts, or even before. Once you have bleeding into the brain, tranexamic acid cannot remove that. In severe patients whether bleeding has already happened, it is too late. So we didnt see a benefit for patients with a severe head injury, but we need to get on with a mild and moderate patients as quickly as possible because we saw a 20 reduction in deaths in those patients. To come back to your last point, how quickly do you think you can move to this being used clinically. You have done the trial, i know you want this put into action quickly but, in reality, how long might that take . I think things are moving quite quickly because the people responsible for guidelines in the uk already know the results. There is already very positive feedback from them about wanting to implement this, and we see this drug being used in ambulances, so as soon patients have their injuries they can receive this treatment. I think getting to hospital is almost too late. Professor Haleema Shakurstill from the London School of hygiene and tropical medicine, thank you very much. I think that there are a number of questions we want to put to the bosses of the business. For example, over the last 12 years, the last three chief executives were paid a combined total of £35 million. Will they be handing any of that money back . And how is it fair that people like that earned multi million multi million pounds, worth of pay and bonuses and yet people like betty are those that end up picking up the pieces. And so those are the sorts of questions that we are wanting to put to the bosses today, as well as whether they did enough to save the company and why they ended up ladling the company with so many millions of pounds, more than £1 billion worth of debt when the company went bust. How is that responsible, and was it ever possible, really, to turn around a company like thomas cook when it had so much debt, which would have been accrued by the decisions of the people running the company, the people that earned so much money. At the start of the hearing, the former ceo of thomas cook reiterated his apology to everyone affected by the collapse. You heard me probably saying that already, but i really wa nt to saying that already, but i really want to repeat it in front of the members of the select committee, how deeply sorry we are that we couldnt save this iconic brand, this company who has a long, long standing history in this part of the uk industry history. A perfect storm of increased demand and Staff Shortages means that people with learning disabilities in england are still being locked up in unsuitable secure units. Thousands of people with learning disability, many children, in some cases are being locked away in rooms in seclusion, being fed through a hatch, being pumped full of drugs to keep them quiet and facing physical restraint. Appalling abuses of peoples rights are taking place in the Care Quality Commission is calling the nhs and the government as on that. These people would be best helped by being looked after and being helped in the community . Thats right, but we need the right services in the community, and there has been a number of government programmes to put that support in place in the community, it has not been achieved yet, new targets have been achieved yet, new targets have been set out by the nhs to achieve that. Why hasnt it been achieved . It is an issue of health and so services having to Work Together to put the right support in place, that has not happened yet. We have people in these hospitals waiting to come out, they do not have the support ready, do not have the right housing or the Skilled Staff team is ready to support them, there is a huge amount of work to make sure there is amount of work to make sure there is a supporting pace to support people in the community. Is it about the demand, is it about the Staff Shortages, is that one of those he would pick out as the nub of the problem . Half a billion p is being spent on buying the wrong type of care. The money is there, it needs to be spent in the right way.
