Joining us is cnn chief medical dr. Sanjay gupta and ali mukdad from Health Metrics and evaluations. Sanjay, on the news overnight President Trump retweeting this message with firefauci. Your reaction when you see that . You know, this Inflection Point between politics and Public Health, i mean its an uncomfortable place. No question about it. I was a little surprised, i think that dr. Fauci clearly is somebody that the country, maybe the world has gotten to know, especially over the last few months, but you know, people in the Public Health community have known of him for decades, as you mentioned, john. Hes worked for several president s and then handled some of the biggest Public Health sort of stories of our time. Hiv aids, but also sars and mers and h1n1 and anthrax and all these other things. Its a tough spotty think. I think sometimes dr. Fauci sort of gets pinned down, these are tough questions to answer, because at the time, we were dealing with a novel coronavirus, so exactly how it was going to behave, what the trajectory was going to be, there was lots of models out there, and it was something he had to balance. So i will say this. I dont know that theres a clear replacement for someone like a dr. Fauci. Ambassador birx was a student of his. Shes obviously at these briefings all the time as well, but she is he was clearly the lead, has been the lead, so i think it would be a real loss. I dont think its going to happen personally. I dont know if this is just saber rattling but i think it would be a real loss if it did. Lets move on to the models that sanjay was talking about, professor mukdad, these are your models, weve been relying on the university of washington models. Tell us where we are today. What are the latest models showing you about where we are on the curve, what places the virus has peaked, and where its headed. Good morning. So we are right now as a country the peak has happened april 11, and were coming down as a country, but several states are behind, and what we are seeing right now is kind of two epidemics. One epidemic in states that issued social distancing early on that will go down early may and second epidemic in states where we will see june so this is the problem, is we have like two countries right now within one country. One country will say i control the infections and can be ready for opening business whereas another part of the country is behind and we are frayed that people who come from one place to another being a country like us where people travel around. The issue is what happens when these two countries are in fact the same country, the United States of america, which gets to something that dr. Robert redfield, the director of the cdc, is saying this morning, sanjay, he says its important to look at the country as many different separate situations. He said that this morning, as he was asked questions about the potential of reopening in may. So how accurate is that . Can we look at america as many different situations and can you do Different Things in different parts of the country . I think thats going to be challenging. Somebody once said during a previous pandemic an infection anywhere is an infection everywhere. Maybe thats overstating the point but that is sort of a lot of people in the Public Health Community Approach this, can you cordon off certain areas and say youre a safe zone because people do move around so much, if not throughout the whole country, then at least throughout the region, so up there, for example, where you are, john, would it be not just new york but new york and connecticut and new jersey, and other places to really create a zone where you think its now reasonable to start getting people back to work. I think its going to be challenging, just knowing how people do move around, and the fact that the virus moves around. Even if the virus, even if people are being sensible, and so many people are being very sensible nowadays, the virus can still move around because it is a contagious the virus is still a contagious virus. Someone said to me over the weekend im reading reports says that the risk that anyone could contract this is low in the United States. Yes, thats true. Thats because we are doing something in this country that we really havent done maybe ever in terms of these universal stayathome orders almost universal across the country. This is a unique situation and thats why the risk right now is low, start to open up certain parts of the country and the risk will go back up. We know people will get sick that otherwise wouldnt have at the time that happens. Its a question of how do you balance the risk at that time with the desire to open up the country. So professor mokdad, what youre saying about some places that didnt do early social distancing and that their peak isnt for a couple weeks out yet, so for instance florida, i was interested to see you predict i think, correct me if im wrong, the last model i looked at was april 27th, so the end of this month is the projected peak in florida, and so given that, what about this overarching question of is it safe, do you think, in some places, to reopen the country, stop the stayathome orders in some regions where it has peaked . Thats very true. So you see florida, texas and georgia at the end of april, the peak and start coming down then. We in the Public Health sector we should start rolling out a blueprint for how to open business when it and weve done similar things before. You remember after 9 11, we stopped flying, then we opened and had to go through metal detector, long lines and we have a better way to do it, tsa precheck, people can go faster. The key for us to open our business would be testing. Are we able to test people and check and make sure when they go back to work theyre negative. Thats the key issue. We need the testing to start rolling out plans for people to go back to work. Professor, what are your models telling you of what would happen to this curve if parts of the United States did open too early . If we would have run two scenarios, open by first of may or first of june, and of course, if you start early on in may at the national level, you see it going up again, so there will be a surge and a second wave, and thats very concerning for all of us, all susceptible to this virus and its still here, still around. Unless we have a measure for us, until we have a vaccine and medication for it, the only way we can go back to normal business is to test people and make sure we can trace any infection, follow up and have people stay at home when needed. We need to monitor the situation as we open up in waves, and phases, and certain places in the country keep an eye on the situation and when we see circulation we have to impose more restrictions. Youre saying without the Contract Tracing and the testing, well see the peaks spike back up. What are you seeing right now in terms of the death toll . I know that that has fluctuated or come significantly down. So today, if we say stayathome orders through april, what do you see for the death toll in the United States . If we stay until end of april right now, this is what our models are assuming, stay until the end of may. The death toll is coming down, and in many states we see like by early may, the death toll will be below what we consider an epidemic which is 0. 3 cases per million, so its coming down in the country. Its coming slower in places in the country where they impose social distancing later on, but as a country, were doing very well, and the worst is behind us, but again, we cannot relax until we are sure that we, the virus is notsirk scl circulatin own communities. Just to be clear, sorry, professor, thats through the end of may, stay at home through the end of may. Thats where you see the death toll coming down. Yes, so far thats our assumption, to stay at home, all the social distancing until the end of may and were seeing it working and its coming down. And those are the decisions being made right now, sanjay. You spent a lot of time this weekend thinking about the last three months, because of this New York Times article which outlined many different examples of when the administration was more fairly specifically about the possibility of a pandemic, and the types of things that have happened. What jumped out to you . Theres an objective time line we could put up but ill give you subjective thinking around this time line as i was talking to people at pretty high levels within Public Health both at the state and the federal level. We know certain things. When we started to first hear about this pneumonia cluster of unknown clause end of december. Even that is a little bit taupe interpretation. Now when you look at some of the journals as early as the beginning of december, there were patients diagnosed with that. Nevertheless, if you focus sort of on the next graph where you look at december sorry, january 30th and january 31st, those particular dates were when we first got letters about the idea that there was asymptomatic transmission. That was on january 30th that the new england journal published a letter on that. There was a thought is this real or an isolated case . But there was evidence on january 30th and january 31st, the next day, cdc said that was happening in this country. Now, that is all known. What is subjective is on january 31st, the humantohuman transmission in the United States was a woman to her husband in chicago. They lived together. It was thought to be extremely close contact. Thats the way the humantohuman transmission occurred. It wasnt still clear i think to a lot of Public Health officials, could that be more widespread at that point. But there was evidence of it even going back to late january. I mean, i remember the day, sanjay, we were on the air, we were back in the studio, it feels like a life time atime ag talking about Community Spread, the inkling of Community Spread and how alarmed you and other medical experts were, because of what that meant for these coming months. Hundreds of structures destroyed, thousands without power this morning. One person was killed in cartersville, georgia, northwest of atlanta. All totalled six people are dead so far, with daylight the Search Operations are getting under way. Here is the scene we rolled up on this area here. You can see the power crews working on a street pretty much blocked off by the heavy trees that came down. The fatality that occurred about a block away around 1 00 in the morning when the storm rolled through this part of the state, and a 34yearold man tidied wh a tree fell on his own. Five people died in murray, northwest georgia on the way to chattanooga. Six overall. Four counties impacted. One tornado came very close to striking just outside the city of atlanta, and then of course you Start Talking about the other states that have been impacted, 11 dead in mississippi, and at least one other person died in arkansas, plus the six you have here in the state of georgia. 850,000 people without electricity, and here is the problem, dealing with the search and rescue effort is now at the time youre dealing with a pandemic, it is stretching First Responders, and of course theres a lot of people out and about mixing, no one Wearing Masks that we can see, but in very close proximity. You have to deal with the tragedy as well as the pandemic. Alisyn . That is a really important insight into what theyre going through right now. Martin, thank you very much. So the dangerous tornado outbreak continues to cause trouble up and down the eastern seaboard. Tens of millions of people are under a Severe Weather threat this morning. Cnn meteorologist chad myers is tracking all of it for us. So whats the latest, chad . Alisyn, certainly not over, after 41 tornadoes yesterday, more are still on the ground right now. There are some communities that i need you to be taking shelter now. Georgetown in north carolina. Well move you on down here with the radar so you can see it. From edistow to seabrook to folly beach, not that Far Southwest of charleston, you would be next in line, maybe 20 minutes from now, farther to the south midway island, skidaway, tybee island. Tornadoes are likely on the ground. Im seeing debris in the air and rotation enough in the storms that i need you to be in your safe place right now. There you see charleston, there you see the pink box, thats where the tornado is on the ground. By 9 00 we move you ahead, by 10 00 we take you farther to the north. There will be significant wind in new york city, boston as well. We could see gusts here 60 to 70 Miles Per Hour. That will bring down trees. If you see a storm coming at you right now, think of it like theres a tornado, because there will be winds of 70 to 80 Miles Per Hour in some of the storms, thats almost an ef1 tornado, if it hits your house wrong. This is a big deal still at this hour, john. Chad myers for us, an active situation. Please heed these warnings, if you are in these zones as the storm moves east. The state of mississippi is reporting 11 fatalities after the tornado outbreak. Joining us is mallory white, director of external affairs and mississippi emergency management. Mallory, what can you tell us about the situation in mississippi . 11 deaths we understand. Do you feel that thats a complete count this morning . Right now, that is what we see and reported to us to our counties and we will continue to have assessments come in throughout the day. Like you said, 11 confirmed deaths and 18 counties right now have already submitted their damage reports to us. Multiple homes, apartments, dozens of people displaced from their homes right now. Our next goal is to look at longterm sheltering and were in the middle of the pandemic so there will be some challenges with that but this is something that we started looking at seven to ten days ago. We knew the system was going to come. We knew it was going to be bad. So weve been in the planning stage for longer than a week now. Just so you know were seeing aftermath of the screen and the funnel clouds on the screen as the storms pass through. You mentioned the pandemic, obviously so many parts of the country now under stayathome orders. How has that affected the response . Well, our First Responders are out there, theyre hitting the ground and theyve been working into the night to ensure that everyone is present and accounted for. Right before the storm came out pushed out another package full of ppe for the counties, so they would have additional supplies. This weekend, the New York Times published a sweeping investigation into all of the people around President Trump who were trying to sound the alarm on coronavirus, starting back in january, and why President Trump was so slow to respond. According to the times on january 18th, Health Secretary alexa czar briefed President Trump on the virus. Then on january 29th, economic adviser Peter Navarro warned that coronavirus could cost more than half a million american lives. The following day, january 30th, azar again warned President Trump that the virus could become a pandemic. President trump reportedly dismissed that as alarmist. Three weeks later on february 21st, the White House Coronavirus task force conducted this mock simulation of the pandemic and concluded aggressive social distancing measures had to be implemented. Two days later, Peter Navarro doubled down on his warnings with yet another memo. Two days after that, february 25th the cdc issued a public warning about the threat, which reportedly angered the president. It was not until more than two weeks later that the president finally declared a National Emergency. Joining us now to discuss is one of the reporters behind that story, maggie haberman, cnn political analyst and New York Times White House Correspondent and as well as cnn analyst david gregory. Maggie, well start with you in terms of this thoroughly comprehensive report. All the different people trying to get the president s attention, starting in january, and he declared the National Emergency on march 13th. Why was he so resistant to the warnings . Theres a variety of reasons, alisyn. One is throughout january, the white house was consumed by the impeachment trial the president was focused on preserving the phase one signing of a trade deal with china, and he didnt want to do anything to rattle stock markets, something of his weather vane for his reelection effort. He was getting warnings from a variety of people, not everybody. People on the Economic Team who continued to resist starker measu measures, people in the National Security Council Robert obrien who were forceful in suggesting there should be more done. The president s soninlaw, jared kushner, started out as not seeing this as a huge threat and then once it was very obvious in march, he became more aggressive. The president had direct warnings and what we tried to do in this piece is not just capture the people who were on various sides of this at various points fwou make it clear that there were issues raised directly with the president , where his own advisers were trying to flag what was coming. His defenders say the criticism is unfair, that information was not reaching him or reaching him through people who he had already seen as discredited, such as alexa czar, the health s alex azar, the Public Health secretary. Specific warnings from several different government agencies, whether it be hhs, his trade negotiator Peter Navarro, the nsc. The chatter on the blogs is w. Whchlt o. And cdc are behind the curve. Im seeing comments from people asking why w. H. O. And cdc seem to be downplaying this. Im certainly no Public Health expert, just a dufus from the va but no matter how i look at this, it looks to be bad. If we assume the same case acertainment case as the h1n1, this looks nearly as trance missible as flu but longer incubation period and greater r. 0. The projected size of the outbreak already seems hard to believe. David gregory, you look at the scope of this reporting and the specificity in the New York Times reporting and you see perhaps shortterm political gain put over larger Public Health. Well, and short Term Economic collapse instead of looking at the longer picture, right. I think everything that the president looks at in his presidency is about his political calculation, how it affects reelection particularly at this time and the impact on the stock market, which for him is the weathervane for the broader economy. He didnt want to upset that, what he was wagering this election only. He failed to see that an adequate response would serve him politically, not just his handling of it but of course if it could resolve itself by the time of the election. Theres another piece of this that i think fogoes to the president s paranoia that is unfounded and destructive, his aversion to what he thinks of as an entrenched bureaucracy which he derisively called the deep state. Public Health Officials warned him of this before. The times writes going back to the bush administration, similar pandemic war games. We think about 9 11, we think about a failure of imagination, a failure to imagine what could have happened. Here this was an absolute dismissal of years of accumulated knowledge about how to face this kind of threat. Theres next to k is no excuse. Maggie, i think its strange to see right wing blogs trying to make dr. Fauci the fall guy and one of the things that theyre saying is he didnt know. Theres this interview on january 21st, where dr. Fauci said it wasnt time for americans to panic. That was january 21st. Okay, maybe he didnt know on january 21st. But as your reporting shows over the weekend in this comprehensive investigation, february was a real lost time period. So if people could be excused for not knowing exactly what was coming in the fog of war in january, february we lost a lot of time and certainly half way through march we still lost a lot of time. I think alisyn, putting dr. Fauci in a specific category, dr. Fauci has been the most public facing Health Expert youve seen. Administration officials are frustrated with the number of interviews he does, he has done according to a number of Administration Officials going rogue, and sort of talking to who he wants to talk to. Fauci did say a bunch of things publicly, including in february, that did not suggest there was a need to be a move to mitigation and thats what you see both people inside the administration and these trump ally websites and commentators talking. The main thing we found in our reporting and we highlight this is a person who has had enormous influence on the president has been deborah birx, the Infectious Disease expert brought on to mike pences team when pence was taking over the task force at the end of february. She has been able to get through to the president in a way that others have not. Is she the only person he listens to . Certainly not. But she seems to have a style with him and an ability to reach him that others including dr. Fauci didnt. I dont think its a surprise youre seeing fauci targeted because the president frames everything as a friend or not friend of himself and if fauci is seeing as oppositional hes going to be targeted. David, what does it matter Going Forward . How does this inform the decisions that are made Going Forward . Well, look, and i think theres going to be considerable time for an examination about what happened, what went wrong, and why valuable time was lost, and i think just the other point about this is, so much focus on china and stopping visitors from china coming, which was important and dr. Fauci has said that. That doesnt speak to the bulk of the reporting about warnings about the need for surveillance and ultimately the need for testing sooner. Thats still the issue now. Now were talking about when can the country reopen in some fashion, whether its schools or businesses. Thats why antibody testing, more testing on a scale perhaps a million tests a day are vital, so the same issues are at play here, which is the president cannot satisfy himself with the notion that hes being a cheerleader for the country. His job is to tell the truth, to guide the country through the good and the bad, and yes, to resolve that tension between the economic ravaging of this virus and doing a reopening safely. So all of these issues now, is the team working cohesively . Are they taking all the necessary warnings and not acting precipitously. These are the main issues now. I know that you all have kids and youre all trying to figure out whats next, and where youll get enough income and everything. So nicole, let me just start with you. You and your husband both lost your jobs. You have two kids, a teenager and a 4yearold. So youre at both ends of the spectrum, in terms of stress, i would imagine, and so how has your life changed in the past few weeks . Yes. How . Well, just a lot of unknowns, which is scary, just not knowing how long this is going to last, and whats going to happen really. And what i mean what are you panicking most about . When you let the anxiety kind of wash over you, which i know you said it does some nights, what is it youre most worried about . Just hoping i can go back to work. I know this is going to take a real big hit on small business, which is what i work for, i was previously working for locally owned small business. So im just hoping that it can, you know, hang in there, so maybe i can come back to that or, and get back to normal and get the kids back in a routine. I know school was just canceled through the end of the year, so its going to be different for a few more months for us. That was a blow to so many of us. Yes. Joseph and belinda, i want to bring you in. Joseph, i know a little bit about your story. You are a worker. You have been a worker since you were a kid, and your father took you to work with him on the oil wells. Youre a well technician. You started your own business i think several years ago, and it was going really well, and then no pun intended, it all dried up. So now where do you find yourself . Alisyn, west texas we got hit with a double whammy, we got hit with the rona and we got hit with the oil wars. So you know, us, you know, we decided to pivot. The oil and gas industry isnt the only industry in the world and four weeks ago i was a well technician. Last week, i wrote my first hit single quarantine lover and this week im being interviewed by cnn, so you know, all this is, is an opportunity to pivot to something different, an opportunity to spread joy, love and peace and hope and thats what im trying to do. 19 cents out of every single song is going directly to the doctors and the nurses that are on the front lines fighting this rona and you know, thats what i decided to do, and i know a lot of people are afraid, a lot of people are struggling, a lot of people like are, have anxiety but im here to spread a little joy, a little love and thats basically how im responding to it. I cant crawl up in a little, you know, ball and cry or put my head in the sand, man. Its time to go. Its time to be proactive and time to pivot to something that you know can, you know, meet the challenge. We like your attitude. I hope it is infectious for all of us. Appreciate it. Everybody copes with it differently and youre clearly a man of action, and you have pivoted to writing songs, which is beautiful, but belinda, you have two little kids and youre pregnant at the moment. Yes. So has that added to your anxiety about the future . No. Im actually, i dont really have any anxiety. We have a very strong faith in god and know hes going to take care of us, no matter what. I see the end in sight and know it will be better than we could have ever imagined and in regarding my pregnancy, i do think, you know, pregnant people are kind of forgotten, but you know, we are very blessed. I have a midwife. This is going to be my second home birth, so its actually falling right into place. I mean, i am ale sure a lot of pregnant people wish they could be at home and ive heard just stories of women having to go by themselves to the hospital to do this, and i cant imagine. Yes. That is a scary scenario, youre so right. I want to bring nicole back in. Nicole, when you hear the upbeat nature of the normans, what is it that has most changed for you . Are you whats happening with feeding your family and have you gotten a check yet . Weve understood that unemployment checks have been deposited in some peoples accounts now. So do you know if thats happened yet for you . Yes, i just received my first unemployment check last week, so that was good. After three weeks of unemployment kind of being a nightmare, i was glad to finally get it. Were glad that you got it. In terms of i know youve had to get creative with feeding the kids. Yes. You can share any hints for other people . You know, were just trying to get creative with cooking and try to, you know, keep the budget, be very aware of every dollar right now, because we dont know how long this could last. So were just trying to be proactive and stay ahead of everything. Look, its the uncertainty, thats what people are struggling with the uncertainty of not knowing how long it will last. They are not Peer Reviewed studies so whether theyre showing benefit of these medications or in this case showing harm of medications, we have to take these with a grain of salt. We do need larger trials. Let me just be clear about that. What this particular study out of brazil and preliminary study, not Peer Reviewed, they had 81 patients, divided into two groups. One group got a lower dose of the medication, the chloroquine which was 450 milligrams twice a day for ten days, another group 600 milligrams twice a day for ten days. What they found was people who got the higher dose were the ones that had these significant side effects, john. There were patients who died in that group that were receiving the higher dose, and there were patients who had these cardiac arrhythmias, these heart rhythm abnormalities. Again, very small studies, very early studies, and we dont know with some of these patients who died, did they die related to the medication or Something Else . Were not sure. They are starting to get i think what this says more than anything else is before this, we didnt really have an idea of what the dosing might look like, how frequently the medication might be given, and now were starting to get some early indications that the higher doses here are potentially problematic. This is a medication that has been used for decades for people for things like malaria, but at lower doses. So we got to figure that out, john. Its not totally to throw cold water on the chloroquine idea but it is another sort of cautionary tale here.
