And, frankly, its been hard to know just what to believe. As President Donald Trump and First Lady Melania Trump have both tested positive for coronavirus. The president has mild symptoms. He continues to be not only in good spirits but very energetic. The president , out of an abundance of caution is going to be heading to Walter Reed Medical Center. We have not heard anything from a president who usually is extraordinarily communicative. This historic moment is defined by what we dont know. Nbc news has learned trump has a lowgrade fever. He does not require supplemental oxygen and has completed his first dose of remdesivir. A single 8 gram dose of regenerons experimental polyclone polyclonal antibody. Has he ever been on supplement supplemental oxygen . Right now he is not on i know you however the timeline is getting to be more and more of a mess here. 72 hours into the diagnosis now. If one does the math, that would put it back to wednesday morning. The white house was actually quick to correct, clarifying that the president is on, quote, day three of his illness not a full 72 hours. The white house travel pool saying the president s vitals over the last 24 hours were concerning and the next 48 hours will be critical. Yesterday we were real concerned with that. He had a fever and blood oxygen level that dropped rapidly. Over the course of his illness, the president has experienced two episodes of transient drops in oxygen saturation. Last night his doctors said he didnt receive any supplemental oxygen on saturday. Now we know he did receive that on friday. Whats the latest on the president s condition and who is telling us the latest on the president s condition . Were still missing a lot of basic information. Lets kick off this hour with nbc political Reporter Josh Lederman live now outside walter reed in bethesda, maryland. Josh, first of all, that motor aid that came through, im sure that created quite a styrene though the president tweeted in the video that he posted that he was going to do it about a minute before he apparently left the hospital. Reporter thats right, joshua. A lot of his supporters here had not had a chance to see that video in the time between it was released and the president s motorcade came out of the hospital and drove down Rockville Pike just behind me past throngs of President Trumps supporters in an unexpected joy ride by the president , who is currently suffering from coronavirus and is symptomatic, which we know from all of our doctors and medical experts is the time when someone is extremely contagious. The president traveled in his president ial vehicle with the windows sealed up, meaning he was in the vehicle with drivers from the secret service. They were Wearing Masks during the period of time when we could see the president briefly through the window, but this is raising profound questions tonight about whether the white house has any real commitment to taking the steps that they have said they are going to take to make sure that the coronavirus does not continue to spread, that they do not put more people at risk. Its not like this isnt something that weve already seen play out. Multiple people in and around the white house now have contracted coronavirus, so much so that the d. C. Government has said that rose garden event a week ago, where many of the attendees have now come down with coronavirus constitutes the Biggest Community spread that washington, d. C. Its cringe worthy exactly for the points that have been mentioned already. Its literally an extension of, you know, probably one of the most dangerous environments you could be in inside the hospital. Remember its not an average suv that the president is driving as part of the motorcade. Thats a pretty fortified driving unit that, in an ironic way, mimics those negative pressurization ventilation rooms in a hospital. It blows my mind, to be honest, because those people, those secret Service Agents, and i couldnt see exactly who else was there, but those Service Agents, who are Wearing Masks, thats not enough protection. And that just concerns me for their health. It concerns me that this is still a president and a white house that is using an extension of a very serious illness and motorcade ride as part of like pr and marketing. It seems like were seeing this recurring theme of trying to make things sound better. You heard that from dr. Conley not just today but yesterday as well about updates of the nt president s decision. Something that was said on american voices regarding one of the medications that the president is being treated with. Watch. When we put patients on highdose stroisd like that, we monitor them very closely. Theres a host of side effects, including impact on their Mental Health and decision making. You can see delirium or psychosis. You are generally not yourself. There comes a time when patients on it very specific treatment regimen maybe shouldnt be making decisions about what they have capacity to do. Shes referring to a medication dexamethazone. What do you make of her comment, with the clear understanding that none of us has reliable information about the president s Mental Health state so were not going to try to diagnose the president tonight, but tell us about dexamethazone and what shes referring to here. Absolutely. Its a very common medication. This is clearly dexamethazone is a steroid, the doses of which has been studied this drug has been studied and shown to be effective in the treatment of covid19 patients, but very clearly through the evidence that we have for patients who have either been on mechanical ventilation, on intubation or have been on oxygen, dependent on oxygen and or have been in the hospital for longer than seven days. And weve already heard, and we dont have all the pieces we need to understand, but we heard the president was on oxygen. We also have to understand that there must have been a progression in his illness which caused the doctors to put him on dexamethazone. Commonly used steroid. I described it the other day for a patient that had an allergic reaction but at a much lower dose. The doses that the president is receiving are higher. The director is directly referencing that on those doses on any dose but on high doses especially, theres a much higher likelihood of side effects and side effects that can occur with dexamethazone include depression, psychosis, suppression of the immune system, weight gain and predisposition for high blood sugars or Something Like diabetes. Quickly before i let you go, weve gotten a few more answers from the president s medical team. What is the biggest lingering question for you in terms of how the president is doing . Incredible questions that remain. Number one, what exactly are the president s viral loads . How much virus is in his body . Theyre drawing blood, looking at what level of infectious virus he has right now. Are those levels rising or declining . Thats important. It els us where he is in the course of illness. Number two, what are the results of those pulmonary scans that we know theyve done . Has there been evidence of damage to his lungs, which is incredibly common in covid19, even in healthy patients that are not hospitalized. Three, the next phase, this critical phase of the hospitalization, even if hes discharged back to the white house residence, this inflammatory phase that can get aggressive and result in your body going into a multisystem, inflammatory state. You can tell that by looking at certain lab markers and we would really need to know what those are. I do think that those will be critical pieces of information for taking care of the president Going Forward and for the American Public to understand, is he on the recovery trajectory or is this still a critical watch and wait period, which is where i believe he is at . I was trying to reflect the upbeat attitude that the team, the president , the course of illness has had. I didnt want to give any information that might steer the course of illness in another direction. And in doing so, it came off that were trying to hide something, which wasnt necessarily true. So here i have it. Does the white house have a point when it comes to remaining optimistic about the president s health . Is it an issue of National Security . I would like to get your opinion on the National Security implications of this, regarding that motorcade we just saw some video. We spoke to one of the pool reporters and said of that motorcade, quote, appropriate precautions were taken in the execution of this movement to protect the president and all those supporting it, including ppe, personal protective equipment. The movement was cleared by the medical team as safe to do, unquote. He did not answer questions about whether the driveby happened at the president s request, whether the president met with anyone in person or why the pool reporters were not notified and called back to cover this driveby. These all raise so many more National Security questions than we had already raised. This feels like the kind of thing that needs to be buttoned up oh so tight lly, because the are so many things that could go sideways. Youre not kidding, joshua. In tackling this whole National Security question, i think it bears remembering just having donald trump as president is a threat to the National Security of the United States and its not just democrats im talking about. Im talking about his former defense secretary, jim mattis and former National Security adviser, john bolton. He doesnt read his intelligence briefings, doesnt understand americas role in the world, the role of nato. Thats the backdrop. Now we have a president suffering from a lifethreatening illness who may, as you say, be on drugs that render him not of sound mind and body. We have this thing called the 25th amendment that has been used by two other president s in our history, Ronald Reagan when she was shot, and george w. Bush, when he had a colonoscopy, basic procedure where he was under anesthesia for a few hours, he transferred power to dick cheney. Why thats not been discussed in terms of Donald Trumps treatment right now, temporarily transitioning power to Vice President pence is beyond me. He seems to be fine in the videos were watching, seems to be mentally sound in those videos. But any time the president of the United States, the commander in chief, the man who has the nuclear football, the ability to rain down destruction on other nations is suffering from a lifethreatening illness, thats my concern when adversaries would try to take advantage. Any number of adversaries from russia, china, iran, north korea, who may well seize this moment to try to do something and we have a president who is under treatment in the hospital now and a Vice President who is not in charge. Its a pretty worrisome situation. Ken, is it safe to assume there are provisions being taken to ensure the continuity of control in the Administrative Branch despite what may happen to President Trump . There are measures being taken, right . There are a continuity of plans always being examined. 9 11 injected a new life into them. Of course, the fact that mike pence has repeatedly tested negative, that takes a lot of the strain off of the continuity of government plans. The real concern becomes what if the president and Vice President are incapacitated and we have to Start Talking about secession act of 1947 and nancy pelosi, and some people think that law is unconstitutional. The Intelligence Community and military chain of command, absolutely thats on autopilot. If theres a threat to the nation, the president has to make a decision about how to respond. He took a brief trip in his motorcade along with other people so he could wave to his supporters gathered outside the hospital. Its an interesting contrast of the first lady showing some caution in this process while the president himself, you know, leaving the hospital after being diagnosed with covid in a car with his secret Service Agents and the driver definitely not social distancing, you know, and putting those folks at risk here, too. He has been positive for the past couple of days now and very well could be contagious and very well could get others sick who he is close with. Its a little bit of a twofold situation going on. We havent heard anything from the biden harris ticket in terms of response on that front. Its possible that they do make comment especially with the Vice President ial debate coming up this week, like you mentioned. What more have we heard in terms of safety precaution . Simone sanders, who works for the Biden Campaign, was pressed on another network about the rate of testing thats going on. She said the candidates are being tested but how often is often . Is it every day, every other day, et cetera . Also joe biden and Kamala Harris technically should not be traveling because if you have been exposed to someone who has covid19, youre supposed to quarantine for 14 days. So what do we know about the Campaign Safety measures now and how much theyve adapted to the president s infection . Sure. Thats a great question. Look, this is a lot of moving parts. Theres a lot of updates going on at the same time. We do know before joe biden travelled to michigan, he took two covid tests. Both of those came back negative. In the same sort of time span, both Kamala Harris and her husband, doug emhoff, took covid tests and said those came back negative, too. We have been seeing some Clear Communication from the biden harris ticket on making sure those tests have been done and are coming back. Like you said, weve known before that the ticket and both biden and harris have been taking tests regularly. What exactly that means, is that every day, is that every other day, we dont know. We do know as of the past 48 hours, the Biden Campaign came out and said that every covid test that the former Vice President takes will be released to the public. They will tell us those results. In these coming days as were seeing things happening and, of course, traveling, it doesnt seem like theyre ramping that down. It seems like both joe biden and Kamala Harris will continue this travel. Weve seen them take these precautions while theyve been on the road, even just from our perspective, socially distant bubbles, all the events have primarily been outside. There are many people who have had covid who have needed oxygen. I dont understand why this lack of transparency. Why downplaying this . You actually understand the details of my district. There are janitors, food service worker, bus drivers. Those folks arent employees that are independent contractors. With independent contractors many times they dont have health benefits. One thing thats staggering, the president , you saw all the treatment he got. I wish that were the case for most americans, every american. If they got sick that they could get that health care. A lot of people in my district that thats not the case for. By the way, theyre often the most exposed because theyre out there doing work. The president says he couldnt just be holed up in the white house. A lot of americans cant just be holed up in their houses. They may not be president , but theyre the ones who are stacking Grocery Stores or driving and doing essential deliveries, and there has been no focus on them. We finally have a bill. We have passed this. Its still not sufficient. Now you see the president of the United States, desperate to pass a more liberal bill, a bill that Speaker Pelosi wrote. No wonder folks have been asking whether our country has a mechanism for providing stability at times like this. Indeed, we do, including the 25th amendment. It lays out the steps for a transfer of power if the president is unable to fulfill his or, some day her, duties. The Trump Administration does not appear to be taking this possibility seriously. At least not as far as theyve said. This morning, white house National Security adviser Robert Obrien was asked about the president transferring power. He said that it is in his words not something thats on the table at this point. Msnbc legal analyst Danny Cevellos takes a look back at how past president s have used the 25th amendment. President ial line of succession is the president , Vice President , then to the speaker of the house. It doesnt necessarily take incapacitation for the 25th amendment to provide. It was a little vague so congress rewrote it into the 25th amendment in the 1960s. It covers several situations. An easy case is Section Three where the president voluntarily but temporarily transfers power to the Vice President. Its been used before. For example, president s reagan and george w. Bush transferred power to the vp while they underwent surgical procedures. More complicated is section iv, involuntary of transfer of power. Its initiated by the Vice President and majority of the cabinet who transfer, in writing, to congress their opinion that the president is unable to continue his duties. Once they do that, the Vice President automatically takes over. Its never been invoked and its designed to be difficult to invoke. Remember the Vice President and the cabinet are likely to be loyal to the president , so they will not invoke this provision lightly. The easy case under section iv would be Something Like the president in a coma. No one could honestly oppose the idea of transferring power in that situation. The harder situation is anything less than a president ial coma, where the president is awake or he recovers and wants his power back. A conscious president can come back and say in writing or in a tweet that his inability has passed. The vp keeps the president ial power for up to four days and he and the cabinet can disagree in writing to congress but otherwise the president gets the power back. But not quite making it succeed for him. What it is about him or his situation making it hard . Its really been tough. It was a disaster for him in the spring. Its gotten better in the fall. Its not just the technology or the wifi connection. Its really the community contaminati communication between the teacher and students. The links and codes necessary to get kids into class and get the work done on the programs. Thats tough. That basic communication. And then just to have the support at home. Having one at home able to follow along and make sure a kid is logging on and they are doing the school work. Thats really the issue for him. He has a large family crowded into a house. Each person in the family is not really in a situation to be playing that role. So hes really been quite a drift in isolated during this period. Without really any real contact with kids his own age. Its not good. So many kids like him. The piece was to show what the reality is. Its worse than were admitting. Talk about systemically the challenges. I can relate somewhat. My mother works with at risk kids and doing the Remote Learning thing. Which is no small task. I understand the Teachers Union argued against reopening because it can put the most disadvantaged kids at physical risk. Remote learning comes with all kind of different challenges that can make education tough for a whole host of other reasons. It kind of feels like a rock and a hard place. Absolutely. What were seeing now in baltimore and other cities is incredible disparity. We in the city have kids in private school going to school. And kids Public Schools are not going to school. Step back and think about that. Its extraordinary. Its like something from a different part of the world or different time in history. Thats whats happening now. Thousands and thousands of Public School kids in the city who are many of them isolated in darkrooms by themselves with a screen all day. And meanwhile we have private school kids or kids in wealthier districts who are going to school and getting in Person Instruction. Its amazing. Half of as many black and hispanic kids are getting the chance to go in Person Instruction. As white children. At this moment. What do you think is the biggest thing to improve the system . Especially since President Trump and education secretary pushed hard for in Person Instruction to reopen. Whats the one lowest hanging piece of fruit in terms of what can be done to improve things now . To be honest i think the base has gotten so politically polarized and its about politics now. We have to wait until the election. When the election comes and donald trump loses you might see a complete change of mind set in a lot of heavily democratic cities and states about going back to school. They see it as a trump thing in places. Its polarized. Its a whole level of distrust around the issue of going back to school. Thats what could make the difference.
