Federal officials found that a window cleaning company is responsible for an employee’s fatal fall from a building in downtown Boston.

The tragic incident occurred just before 8 a.m. Monday.

A Window washer in Boston died after falling from a 32-story tower at 100 Summer Street, which is one of the tallest buildings in the city.

The father of two fell from 100 Summer St., one of Boston's highest buildings.

Investigators from the federal Occupational Safety and Health Administration continued their probe Tuesday into a 40-year-old window washer’s fatal fall from a downtown Boston office tower the previous morning, officials said.

On Monday, workers from Sky Safety, Inc. were window washing in downtown Providence at 100 Weybosset Street.
