He said he felt the matter 'hasn't been looked into properly' and the fact that had yet to happen three years on 'I think that's pretty poor'. 
'I think more action should've been done within the first year - looking into this situation and what happened and why it happened - to prevent this from ever happening again,' Mr Cockman said.  
Western Australia's coroner Ros Fogliani ruled out a public inquest into the deaths stating it was unlikely to generate further evidence.
