New york city is now home to roughly a third of coronavirus cases in the United States. And the mayor is fearful about whats ahead. Were about ten days away now from seeing widespread shortages of really fundamental supplies, ventilators, surgical masks, the things that are absolutely necessary to keep a Hospital System running. New York Governor Andrew Cuomo asking President Trump to nationalize efforts to produce supplies. The president can say to the manufacturers, i want you to make these products because its a matter of Public Health. Officials say the demand for equipment like masks and ventilators is making it hard to provide hospitals what they need. Were competing against each other and against other countries. Its a wild west i would say out there. President trump would say its primarily up to the states to create their own coronavirus responses. Were sort of a backup for the states. Some of the states are doing really well and some dont do as well. This morning, at least eight states issuing state at home orders. In parks and beaches across the country, some americans are still crowding public spaces. Theyre trying to avoid using Law Enforcement to prevent people from gathering in large groups. Brynn, thank you. Millions of americans, meantime are struggling with layoffs, furloughs and pay cuts and turning to the federal government for help. They remain in limbo as the senate hits a major roadblock on a stimulus plan. Joe johns is live with the latest there. Joe, good morning. Good morning, erica. It is not over yet. It did fail on a procedural vote last night in the senate. But theyll be back at it again today. The senate back into session around noon eastern time to try it again. Tempers are flaring during the negotiations for what could be the largest stimulus package in the history of the United States government. 1. 8 trillion. The arguments are going along predictable lines. Democrats arguing that this package does not do enough for families and workers and republicans arguing that if the package helps companies, at the end of the day, it helps workers as well. There is a lot at stake here. Especially during an election year. There are great concerns to the United States senate and all on capitol hill that the Congress Needs to do something to help workers and calm the market. John, back to you. All right. Joe, thank you very much. Were also getting word, joe. You can help us understand more that dr. Anthony fauci, the top Infectious Disease doctor who has calmed the nerves of so Many Americans and for generations that he may be getting frustrated with some of the things the president is saying. Explain. Certainly an indication, Anthony Fauci was interviewed for the journal of science and was asked about the president s penchant for exaggerating, some have called it dhecheerleading, some call it misrepresentation. He said essentially that the he cant jump in front of the microphone and push him, meaning the president , down. He said lets try it again and get it corrected next time. Fauci, the peace maker there. But also watching the controversy behind the scenes here on the Coronavirus Task force. Theres still clearer question b. Behind the scenes about how much the president is telling the American Public and how much of that needs to be corrected by other members on the team. John . Exactly how i like my coffee. Youve got your carafe. Ive got my light roast. Were brewing the love. Developingover night, President Trump activating the National Guard in the three states hit hardest. New york, california and washington. It comes as governors plead with the federal for supplies. Dr. Daniel vargas, chief Physician Executive at Hackensack Meridian Health. Hes leading the medical response in new jersey. Arlene van dyke, Critical Care nurse at the Holy Name Medical Center in new jersey and Cnn National Security analyst, no julia. I want to start with you, julia. What exactly does calling in the National Guard mean and what do they do . It means essentially their status changed from state active duty which is the governors control them and tell them what to do and pay to whats called title 32. It is just a way for the governors still to be able to utilize them. So theyre not under trump or the secretary of defense. But the feds pay. General lit status is, when theres a National Security crisis, we used this after 9 11 when the National Guard was used to protect airports. Its just a money shift. It relieves the burden on the states. Its good that it was extended to those three states. But President Trump probably should prepare to extend it to all 50 states so that governors can begin to get their National Guard ready, building things, preparing, helping with logistics, supply chains. All the things that the military is really good at doing to relieve the stress on Public Safety and Public Health that are under tremendous stress right now because of patients. Erica . Whats still to come, dr. Vargas, as you look at this and youre working on your preparations, how are you trying to game out this timeline so that you can make sure youre prepared all the way through . Well, i mean, right now the four biggest areas are clearly going to be testing, masks and supplies and making sure the supply chain is there. Staffing is a really big issue very soon and then also capacity management. Were right now at critical levels relative to the timing of getting tests done. Relative to our access to masks. We have probably three to five days of n95 masks left. Were already reinstituting all sorts of different staffing models to be able to stretch the highly skilled nurses that we have right now across as many patients as possible. Were reactivating almost every day. Arlene, what are you seeing . Youre on the frontlines interacting with patients as they come in. What has that been like . Well, you know, as we are all aware, these patients are extremely critical ill. This care becomes very challenging, very stressful for those of us at the bedside. Theres obstacles that we become creative in trying to get around. Then we have the emotional aspect that we are all dealing with. These patients are on ventilators, theyre on multiple iv drips and medications, sometimes anywhere between five and ten drips. Along with other medications and physical care that needs to be provided. So its extremely challenging and stressful. Can only begin to imagine. Julia, as we look at what could be done, theres so much talk about the defense production act and the president having not really formally invoked this despite him going back and forth on it. Theres a lot of purporting in the New York Times and the Washington Post about companies being confused as well. What should happen at this point . What should and shouldnt happen. If i could talk for a second and thank my other panelists for what theyre doing to protect all of us. Its remarkable. The defense production act is actually genius in the sense that it allows a president to not simply invoke, thats what fr trump has done, but turn on manufacturing. Basically, would you say the president would say to companies, look i need half a million ventilators or a million masks, whatever the number is, then they would be certain that there was a purchaser. Thats the point of this. What youre seeing in the reporting is these companies are saying, i can switch my manufacturing, i just have not been guaranteed a purchaser. Im not wasting my time building something where theres not a purchaser. The defense production act prevents price gouging so that the basically what were hearing from the governors is theyre competing with each other rather than a unified approach. What would be the better thing is that donald trump actually turns on the production act and starts surging these materials we know we need. Pays the private sector and the private sector is happy because theyre satisfying the National Interest and getting paid for t it is the simplest most genius act coming out in response to the korean war and why the president isnt moving forward is something to be said. Something like venezuela, this is not a taking of the private sector, this isnt taking over manufacturing. Its just prioritizing the production schedule and guaranteeing to the private sector they actually have a market and it ought to be used. It should have been used two months ago when we knew what we would have. Were hearing that the private sector is ready. Theyre ready. They need an order. Lets be clear. The president is talking about the act but he is not using it yet. He could. Thats a choice right now . I described it as hes plugged in the microwave but hasnt pressed start. Thats all hes done. Whats the use of having it plugged in if youre not using it. Thats his choice. Were waiting on that. Dr. Varga, you say youre spending a lot of your time creating space, opening up beds. Creating systems to take in, i think, what you consider to be a wave that is beginning to come right now. Tell us how youre doing this. Creating the space. Why. And also the triage tents that youre going to erect outside some of the facilities. Thats right. We have multiple things were doing right now. Weve erected tents outside the facilities which are largely for triage and infectious patients as they come to the hospital. We also relocated two mobile Emergency Rooms to two of our largest campuses and those are actually in place right now. Some of the most amazing stuff ive seen, though, is were turning regular medical surgical units in our hospital, Hackensack University Medical Center has almost overnight is turning med surge units into icus in creative ways. Cutting holes in doors and putting windows so nurses can see the patients. Theyre putting big filters that help ventilate contaminated air outside and literally were filling them up as we bring them online. As were hearing about some of the changes happening at dr. Vargas hospital, arlene, were talking about what you are facing on the frontlines. How much input have you and your colleagues had in terms of the changes coming your way as you adapt, im imagining on an hourly basis, not just a daily basis. Right, as i mentioned, every day were dealing with obstacles that were becoming very creative in overcoming. Our administration has been extremely respective and responsive to things that, as Critical Care nurses, weve come up with different ideas that can be done. If its feasible and safe, they do it for us. Pretty much whatever we question or ask, they are very compliant with that. Our hospital is going through changes every day. They are creating rooms of negative pressure. Our icu went from two negative pressure rooms to 19 negative pressure rooms within a matter of three days. Were constantly 24 7 expanding our capabilities to accept more patients and be safe for them. I want to read you something that United States. Its monday, march 23rd. Were constantly 24 7 expanding our capabilities to accept more patients and be safe for them. I want to read you something that President Trump wrote overnight and i want your help in understanding what it means and the significance Going Forward. He writes, we cannot let the cure be worse than the problem itself. At the end of the 15day period, well make a decision as to which way we want to go. We cannot let the cure be worse than the problem itself. I will tell you, thats something you hear out there in the world right now. By shutting everything down, were creating problems that may not be as bad as if we let coronavirus run its course. There seems to be a suggestion that the 15day period may be it, its hanging in the balance. What do you say . Its a surprising tweet. Let me just make it clear. No country has done this in 15 days. Not even the most aggressive totalitarian countries. The 15 days was a way of sort of making it easy on us. Were not doing aggressive nationwide social distancing, so its a bit of an experiment. Social distancing buys us time. What does it buy time for . On the far end the vaccine which no one thinks will be distributed for 18 months. But well before that, theres going to be mitigation measures, treatment, greater capacity to help the doctor and the nurse. Theres going to be other ways that we can protect people. More testing for example. Were just buying time. This 15 days is bogus. I mean, its just a number out there. We will see where we are in 15 days and my concern with this tweet and ive been wondering what trumps plans are. He has been looking for a quick fix, a vaccine, treatment. Theres no quick fix. An honest white house, even dr. Fauci trying to prepare us prepare us that this is really hard. In fact, its going to get harder until it gets easier. Weve got to flatten the curve, protect our hospitals and med surge resources. But people who want to get out, right, we have to balance the economy and other needs. We know that. But for people who want to get out in day 15, what they wont answer is what do you do when our Health Care System collapses. Com the number of coronavirus cases in the world topped 340,000. In the United States, we have more than 400 deaths. Cities across the country. Including some of the largest are saying that they are bracing for a wave and they do not have the supplies that they need. Back with us, dr. Daniel varga, chief Physician Executive at Hackensack Meridian Health and Arlene Van Dyk, both in new jersey. Arlene, i want people to understand what it is like for you to interact with these patients who come in concerned. What is the actual interaction like . Well, in the Critical Care Department Im in, were at the frontline where these patients unfortunately, have taken a turn for the worse. They are the sickest of the sick and coming to us on respirators. Emotionally, were dealing with the families who cannot be with their loved ones, cannot see their loved ones and the care becomes very challenging. I was going to ask you about that. These are people who by definition need to be alone, separated from their families. Whats the protocol there and how difficult is that . Right. So emotionally for the nurses, thats a very difficult aspect because in providing quality care, the families are included. In this crisis, the families cannot personally physically be included at the bedside like we usually would do. So ways that were adapting to that is we will put families on ipad, skyping just as like what were doing now so that they can see their loved ones. More importantly, that they can speak to them and feel theyre connected to them even though they cannot be personally at the bedside. Dr. Varga, i dont want you to wade into politics. Its more important that you save lives. But i do want your take on something the president said and what the possible implications of it would be. The president said in a statement overnight he wants to make sure that the cure isnt worse than the problem and that he would reassess this 15day call for social distancing at the end of the period. Based on what youre seeing, what would happen in your hospitals if at the end of this 15day period were only a few days in right now, the president were to say, okay, were done. What if we stopped now . In serms of social distancing . Its hard to say. The one thing you can say for sure is that the spike in cases is continuing and its on its ex o potential part of the curve. We havent seent curve yet. Anything to flatten the curve is going to be incredibly valuable. The way this virus works, its incubation period, et cetera, we dont know who is infected right now. The best thing we can do is to actually maintain the social distancing, wash your hands, not come to the e. R. If youre minimally sick. If you can, take advantage of the Testing Centers that are around so that you know. More importantly than anything, take care of each other. I dont think were going to see even the peak of this for several weeks. Are you set up at this point to handle what you fear might be the peak . You know, its a good question. I mean, i think weve done a lot of things, just like a lot hf Health Systems have done. Weve canceled elective procedures, weve done, as i mentioned a marvelous job of trying to convert space to create Critical Care capacity. My big concern Going Forward is really material and the toll this takes on our folks on the frontlines. These folks are the most collaborative, compassionate, courageous and creative people youve seen in your life. Theyre standing in the breach. Were trying to get them the stuff they need to really manage what theyre seeing every day. But the big issue is going to be material and the ability to continue to support our frontline caregivers. Well find space. Its really more the supply chain and the human cost. Its physically exhausting, its emotionally exhausting. What you are both doing is saving lives and keeping us safe. So we appreciate it. I wanted to thank both of you, Arlene Van Dyk and dr. Dan gel varga. Thanks for what you do. We know how hard it is. Thank you. They remain in limbo this morning in a stalemate in the senate over a stimulus plan. According to one fed official, unemployment in the u. S. Could reach as high as 30 . The Hospitality Industry has been one of the hardest hit. Cnns ken law spoke with people who lost business, jobs and are right now trying to get by. How are you doing guys . Its been a completely life altering experience for me from start to finish. Within a week. This is unbelievable. Like the virus spreading across the globe, the economic damage leaves no restaurant untouched. We would have all of the seats filled. All of these seats. It would be a line out the door. California stay at home orders changed the entire industry in an instant. We went from being about to franchise to basically running a togo business. I havent slept. Im worried about having a heart attack to be perfectly honest with you. With no diners, the theyre hemorrhaging thousands of dollars a day. Restaurants, a sign of whats to come in the u. S. Economy. The industry estimates up to 7 Million People will lose their jobs in the next three months. Nearly half of all service workers. Have a good night. Josh sauter already forced to make that hard choice. I had to was forced to lay off. 75 people. At first youre thinking about them, okay, i feel horrible for them. Then they have to go home and tell their family, i got laid off. I called my wife over the phone and said honey im on my way home and she just she pretty much immediately knew. Laid off from the crowd he miedly filed for unemployment. One of the 2. 25 people who filed jobless claims in the first week. More expected in the weeks to come. Youre talking thousands and thousands of people looking for work simultaneously. Its going to hit every aspect of life. And the government needs to react and help us get through this. Thats the only way its going to work. People are not going to be able to support their families for more than two months. Already signs money is getting tight. Outside the West Hollywood bar employees only a line. Inside, the small staff preps meals. Free meals for workers who show a pay stub. Like this bartender. All of us like immediately lost our jobs, i think, as of monday or tuesday. Are you worried about how long this will last . 100 . If it goes on months like i dont think any of us have any idea what were going to do. Were going to dig ourselves in a hole regardless. Are you scared . Im concerned. Restaurant owner tom is a thousand Dollars Per Day rent. He doesnt want to fire anyone. This is a new reality he will have to face. Yeah. All we can do is help each other. Cnn, los angeles. It is so hard for so many people now. Not a few people. Were talking millions. Yeah. Of people. Christine romans joins me now. Romans, people are struggling with this. People who were working two weeks ago are now not working. Theyre looking to congress for help. Theyre delayed. What should people do . Look, if congress does get its act together, for example the Small Business owners would have a couple of weeks or more of cash flow to retain their workers. Thats one of the things the stimulus plans wants to do. Give the money to Small Business owners so they can pay people not to work so you dont disrupt the economy even more. If you have lost your job or a Small Business owner with bills to pay on april 1st, the first thing to do, john, is talk to your lender. Theyre ready to hear from you. The phone lines might be jammed. Bank of america for example is deferring your mortgage payments, your Small Business loan, your auto loan, a lot of other banks doing the same thing. You have to talk to them right away. Student loans by the way, the president has said interest he waived interest for 60 days on student loan payments and then went further and saying you dont have to pay the minimum for the next couple of months. Check with your lender. Super important. If this stimulus plan passes, john, youre going to get some money. The treasury secretary this weekend said the plan was about 3 grad for a family of four. It caps out at a certain income level. So this is meant for people working people, people who need money to pay their bills if Congress Gets its act together. What its sounding, the idea of 2 trillion might not even be enough. If it is enough or when we get through this romans, people are talking about a vshaped recovery. Give a visual aid. How quickly can we come back . Last night we heard from the president. He said it will be beautiful again. We have to help the worker and save the companies because as soon as were finished with this war, its not a battle, its a war, as soon as were fin wished this war, our country is going to bounce back like youve never seen before. Hes talking about pent up demand. All of the things youre not buying, the things you arent doing now will come surging back in the end of the year. Some economists are hoping thats the way it is. Thats assuming everything is perfect. Thats assuming we get our hand around the virus. That theres money, maybe even more, well over 2 trillion. Maybe two more phases of stimulus gifts. Get it in the hands of Business Owners and people. What theyre fighting about is essentially how do you do you throw money at the companies. Do you throw money at people . The answer is yes. Both. Even then some. Weve never embarked on a rescue. Thats what this is, a rescue of the american economy. The next couple of days are critical. Theyre not trivial questions that theyre held up on in congress. Theyre talking about how much oversight should be on 500 billion. Thats a lot of money. Thats a lot of money. Look, we have recent history of the 2008 financial crisis. The American People were really unhappy about the perception that consumers and workers were not helped as much as companies. So they want some Strings Attached to all that money and some oversight of that money. They dont want a black hole of money to Favor Industries or favorite companies. They want oversight of that. They want robust unemployment benefit, longer and more money. We dont know how long this is going to last. I will say this is revealing something about the american economy. Just how close to the line so Many Companies and so many people were in what was heralded as the best recovery, the longest rekoifr expansion in the history of the country. Really, look how quickly it this coronavirus is so big and unsettling. But people really were living close to the line. Were talking about just weeks. Its only been about six weeks since this came into focus in the United States. Christine romans, thanks so much for your work. Appreciate it. Sure, john. There are more than 341,000 worldwide. More than 15,000 deaths. A dramatic rise in europe, particularly in spain and italy, despite nationwide lockdowns there. Germany meantime has banned gatherings of over two people. That, though, is happening as the country has a remarkably low number of cases and deaths. Is there any insight how theyve managed to keep the numbers down . Right. Comparatively speaking across europe, germany has had a relatively low number of deaths. Angela merkel, the german chancellor introduced draconian measures including banning meetings of more than two people. Making it a rule that people in public cant be closer than 1. 5 meters together. She thanked the public for abiding by the rules. Its difficult to draw conclusions at this point. Where i am in the united kingdom, people have had severe issues with people abiding by the advice. According to angela merkel, germans have been heeding that. Sadly, they havent of course, the government has condemned and sad to see things like that happening. I should also say that the chancellor herself is now in quarantine after she found out that shed been attended to, given a pneumonia vaccine by a doctor who subsequently tested positive for coronavirus.
