What happened and where the investigation stands this morning. it was very violent when the rapid decompression and the door was expelled out of the plane. reporter: new images from the national transportation safety board show the force of the failure on board alaska airlines flight 1282. damaged and contorted seats from a 400 mile-per-hour rush of air through a refrigerator-sized hole ripped in the side of the plane. the headrests on 25a and 26a were gone. the extra oxygen mask was sheered off. reporter: ntsb chair jennifer homendy said even the cockpit door flew open. the time there is a bang, the door flies open. so it did eventually shut, but it did blow open during the explosive decompression. reporter: amazingly no passengers were seriously hurt. ....

So what happened in this case was an unused door blew out of the aircraft when it was around 16,000 feet, and it was a few minutes from take off. so at that point in the flight, all of the passengers would have been strapped in place, they would have had their seat belts on, and the aircraft was not travelling too high. nevertheless, there was still a sudden decompression, we have heard that the cabin door flew open, that the headsets were ripped off the pilots, that phones and other debris went out of the aperture, but if that same incident had happened at cruising altitude, which for the 737 max is about 38,000 feet, the pressure differential between outside the plane and inside would have been much greater, and people would likely. at that stage in the flight they would have been moving around the plane, they probably wouldn t have had their seat belts on, and in a worst case, people could have been sucked out of the aircraft. there would likely have been fatalities. there would cer ....

Large, like a door sized gap in the aircraft, you would have a rapid decompression that would affect all parts of the aircraft. in fact, according to the ntsb, there was damage from several rows behind this door all the way up to the cockpit area. so there was a rather chaotic scene, loose objects, papers and whatnot flying out of the aircraft. in fact, there was a critical checklist that was in the cockpit that was blown out of the cockpit somewhere in the back of the plane. so even if there was not damage, it was affecting how the crew could fly this aircraft. and yet we don t know what the chatter was between the pilot and the co pilot, because apparently the recording, the voice recorder, has been overwritten. is that typical? correct. that s very atypical. the voice recorder in the us is required to have two hours worth of data and apparently after this event was over, rather than cutting the power off to the voice recorder, the power was left on. and over the next two hours, it s ....

Found. yeah. she made the point of how much worse this could have been. no one in the seats next to the plug everyone was seated. i mean, what could have happened if the factors in play were different? well, and we only have to look in history. i ve been working air accidents long enough to remember cases where on other aircraft a door was lost. and in some cases losing a door and this was in the 70s and 80s on these plane accidents, the aircraft was lost. and several of them the plane crashed and everyone was lost. and a couple of the cases of door losses people were sucked out, seats were sucked out of the aircraft. so losing a door is a very significant event. and she mentioned it was rapid decompression. if it happens in under sort of a split second, they call an ....
